Overview:
A Family Peer Mentor (FPM) provides peer support to help families involved with Child welfare where substance abuse is the primary risk factor. The FPM will serve as a member of a Multi-disciplinary team to support and encourage families in their recovery. This position is a full-time (40 hours per week) position located in Butler County and will be a combination of office and community-based work and will involve transitioning clients as needed. Also, this position will manage a case load of 10 to 12 families.
Duties:
- Entering records in the EHR
- Provide assistance with outcome data
- Report and communicate regularly with referral sources and providers
- Provide service documentation
- Participate in case-staffing
- Provide motivational interviewing and support
- Engage in correspondence with Butler County Children's Services and other service providers
- Provide SUD and MH case management
- Perform other related duties as assigned
Qualifications:
- High School Diploma or equivalent. CDCA preferred
- Applicant must be able to communicate effectively in both oral and written form
- Applicant must be able to work both independently and with a team
- Experience with electronic health records is preferred
- Must demonstrate long term recovery from SUD
- Must have lived experience with the child welfare system either as a parent or a child.
- Must have a motor vehicle in good working condition and maintain a current driver's license and automobile insurance
