Case Management Specialist (Female)
INFORMATION
Type of Position: Non-Exempt
Location of Work: Juneau, AK
Salary Grade(s): 8/9
Hours of Work: M-F, 8:00 am – 4:30 pm
Employment Category: Regular, Full-Time
PURPOSE
The Case Management Specialist will provide case management services, coaching, mentorship, and guidance for Reentry & Recovery Program clients. The incumbent will use a wrap-around service approach to provide professional-level casework services.
REPORTING RELATIONSHIPS
Reports To: Department Manager
Supervises: Up to two FTE
Liaison To: Office of Probation and Parole
- Primary contact in addressing the needs of Reentry & Recovery Program participants
- Work with participants to identify strengths and develop Individual Self Sufficiency Plans
- Work with Housing Program Coordinator to plan, develop, and implement group meetings and meals
- Provide confidential case management services including initiating referrals and follow-up communication with other service providers, transport resident to appointments in company vehicle, and attend appointments with resident as necessary in an advocate capacity, etc.
- Responsible for reading, understanding, and explaining policies related to resident services
- Assist the development of a culture of recovery, reentry, and peer support by supporting healthy peer relationships
- Work with participants to develop Individual Recovery Action Plans (IRAP) for reentry/recovery
- Ensure the needs of participants are met through collaboration with Housing Program Coordinator and Resident Manager
- Assist with access to community resources for participants, including public transit, Job Service, Alaska Learning Network, local 12-step meetings and or other recovery support meetings, mental health services, and other community resources
- Engage appropriately with returning citizens to support them in identifying strengths, existing supports, and community resources
- Work with each participant on their financial wellness and create a financial plan. Work with Housing Program Coordinator to ensure that payments are being made and unavoidable hardships are addressed in financial plan
- Interview reentrants and make recommendations as to appropriateness of applicant as a resident of Reentry & Recovery programs. Interviews may occur in correctional facilities
- Maintain a waiting list for future participants and conduct regular in-reach to Lemon Creek Correctional Center for information and recruitment purposes
- Explain program options and services available to the resident, and the requirements of each program
- Ensure participants’ right to privacy and ensure appropriate confidentiality at all times
- Assist clients in completing necessary paperwork, follow-up, and assist client in acquiring necessary documents via a release of information
- Responsible for daily case-noting all contact with participants and service providers using a designated database
- Work with participants in setting goals/objectives for successful reentry and obtaining permanent housing
- Process internal paperwork for the resident in a timely manner to ensure program goals are met
- Work with employers to ensure participants are meeting their employment responsibilities
- Work closely with the Probation Office and participants to support compliance with terms of probation/parole
- Conduct interviews to determine needs

NECESSARY SKILLS AND KNOWLEDGE
- Extensive knowledge of local, State and Federal service agencies and resources
- Ability to read and interpret procedures to make conclusions and decisions
- Mastery of basic office procedures and practices
- Excellent computer skills including all MS office Suite programs
- Knowledge of interviewing techniques/Motivational Interviewing
- Excellent writing skills
- Ability to evaluate and recommend changes in a participant’s reentry plan
- Excellent case management skills
- Excellent customer service skills
- Excellent interpersonal communication skills
- Excellent prioritization and time management
- Ability to practice tact, courtesy, and discretion
- Experience working with Native American/Alaska Native clientele; sensitive to the cultural diversity and needs of this population
- Knowledge and current on approaches to recovery and reentry into the community for persons who have been incarcerated
- Associate Degree
- 2 years experience working in the reentry/recovery field or a closely related position
- Valid driver’s license
- Must be able to work with individuals of various cultures, customs, and backgrounds
- BA degree in Social Services or related field
- LBSW – Licensed by the State of Alaska
- Accredited Case Manager
- CPR/ First Aid/ NARCAN trained or willingness to be certified soon after date of hire
UNUSUAL PHYSICAL REQUIREMENTS OR RESTRICTIONS
The majority of work is performed in a professional office setting and is generally sedentary, requiring routine walking, standing, bending, and carrying of items weighing less than 40 lbs. Travel on small aircraft or ferry may be required.
