GENERAL DESCRIPTION
The floater position will require an individual become very familiar with the various programs and services. The individual in this position must be flexible, adaptable and highly organized.
Serves as a floater for service coordination and other Local IDD Authority responsibilities. The floater position will have consistent job duties assigned, but will not carry an on-going caseload. Cases and activities will be assigned as needed to ensure uninterrupted services are provide in the event of a staff outage or vacancy. The floater will assist in the following areas : service coordination to individuals receiving General Revenue services, Texas Home Living, Home and Community-Based Services, Community First Choice, as well as enrollment activities into those programs. Regular job duties will include completing, or assisting, with transfers, suspensions and discharges. This position will also assist with quality assurance activities.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Does not engage in gossip or negative talk, but instead is focused on solutions and ways to improve morale.Assessment : Responsible for the assessment of consumer needs based on the discovery process using the appropriate tools. Follows requirements set forth by HHS or Center requirements in regards to completion of assessments such as, Service Coordination Assessment, PAS / HAB assessments, ICAP (Inventory for Client and Agency Planning), IDRC (Intellectual Disability / Related Conditions), and Discovery Tools.Service Planning : Responsible for the development and ongoing evaluation of the Person Directed Plan, including facilitating the planning team meeting. Follows all HHS and Center requirements regarding timelines and completion of the PDP.Monitoring of the Plan : Responsible for making face to face contacts with consumers on assigned caseload based on the frequency set forth in the Person Directed Plan. Responsible for meeting the unit policy regarding meeting a contact goals. Documents delivery and satisfaction, and progress or lack of progress for both waiver and non-waiver services at least once every 90 days. Service Coordinators may be responsible for approving, approving and coordinating clients' trust fund accounts.Documentation : Responsible for entering and final approving progress notes into the Electronic Health Record (EHR) by timelines set forth by the Center.Enrollments : Assist in completing enrollments by timelines set forth by HHS.Transfers, Suspensions and Discharges : Completes, or assists in completing, transfers for clients entering into Central Counties Services catchment area, or in-catchment or outgoing transfers if carrying a temporary caseload. Assists program management in reviewing suspensions and discharges and taking action, if needed.Quality Assurance : will work with Quality Assurance Specialist, Program Managers and IDD Project Manager on tasks such as chart or note reviews, running reports and identifying trends, preparing reports regarding goals and quality standards.Communication : Responsible for carrying a work cell phone and returning calls from the office phone and work cell phone by 5pm the next business day. Responsible for checking emails daily and responding by 5pm the next business day. Must contact Supervisor when out for the day or running more than 15 minutes late. Must indicate on calendar where they are going and a general time of when they should be returning.Participate in the Random Moment Time Study as required, including completing the required training.May be required to drive to the different 5 service counties, as needed, to complete job duties. At times may be required to travel outside the catchment area for trainings or for cases with extenuating circumstances.Other duties as assigned.Adhere to the Work Values and Behavior Addendum and demonstrate the qualities listed of : high ethical values, trustworthiness, integrity, cooperation, personal accountability, cultural sensitivity, conflict resolution and problem solving, appreciation and recognition, and respect and dignity.Regular attendance, dependability, and promptness are required for the 8-5 work day or agreed upon hours 100% of the time, to ensure consistency and completeness of program's processes.Professional Development - Successfully complete training required to maintain skill competency and cross training to ensure consistency across program procedures. Complete continuing education as required by professional standards and the Center.Frequent daily driving within the catchment area is required.GENERAL QUALIFICATIONS
Experience / Education / Licensing / Certification
Bachelors or advanced degree from an accredited college or university in psychology, social work or related social services area, OR;Associates degree in a social, behavioral, human services or health related field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice, OR;High School diploma or a certificate recognized by the state as the equivalent of a high school diploma, and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.Knowledge / Skills / Abilities

Documentation must be timely and accurate.Ability to proficiently utilize computer programs, e.g., Microsoft Word and Excel.Ability to see, hear and talk effectively within job requirements.Ability to stand, sit and walk for extended periods.Ability to effectively handle potentially aggressive behavior in a positive manner.Ability to be flexible and pivot as department needs arise.Strong organization and prioritization skills are needed.Relationship Skills
