Job Description

Job Title : Physical Therapy Assistant - Home Health

Job Specialty : Therapy / Rehabilitation

Job Duration : 13 Weeks

Shift : Day, 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : None

Experience : Minimum 1 full year of Adult Home Health Experience is required

License : Must have valid and current license in hand

Certifications : CPR, BLS required

Must-Have :

Reliable transportation
Current driver's license
Agency-required liability insurance
Proficiency in HomeCare HomeBase EMR systems
Must be open to traveling up to 75 miles from the assigned care center
Comfortable with completing documentation within 24 hours of the appointment

Job Description :

Conduct home visits and periodic office meetings

Responsible for visiting patients in assisted living facilities and residential homes

Manage an average of 5-6 patients per day, achieving approximately 6-8 productivity points

Types of cases include start of care and follow-ups

No on-call requirement

Open to picking up additional assignments from nearby clinics if patient census is low

Work locations include various territories within the specified region

Attend weekly meetings at the designated office location

Comply with all documentation and billing standards, ensuring timely and accurate submissions

Ensure flexibility in patient acceptance and maintain travel efficiency

Potential for hire after the completion of the contract assignment
