HOW YOU’LL MAKE AN IMPACT
The Mental Health Practitioner Vocational Specialist: Assertive Community Treatment (ACT) is an active member of an evidenced-based clinical team, providing medically necessary treatment, rehabilitation, and support services to identified clients with severe and persistent mental illness (SPMI). The Vocational Specialist is a key team member in the service planning for clients who want to work or are currently working.
- Provides supported employment & education service, which include the following: (1) Engagement; (2)Vocational assessment; (3) Job development; (4) Job placement (including going back to school, classes); (5) Job coaching & follow-along supports (including supports in academic settings); and (6) Benefits counseling.
- Instruct, assist, and support clients in areas such as interpersonal communication skills, community resource utilization and integration skills, crisis assistance, relapse prevention skills, health care directives, budgeting and shopping skills, healthy lifestyle skills and practices, cooking and nutrition skills, transportation skills, mental illness symptom management skills, household management skills, employment-related skills, and integration into community living.
- Make changes in the treatment plan to ensure immediate and appropriate interventions are provided in response to changes in mental status/symptoms or behavior which may put the client at risk.
- Drive to client homes or various locations within the community where services are provided, and transport clients.
- Establish therapeutic relationships with assigned clients and meet with each client as planned.
- Work collaboratively with a variety of community providers, employers, landlords, and families.
- Provide on-call crisis intervention as scheduled.
