HOW YOU'LL MAKE AN IMPACT
- Performs duties as outlined by The Pact Model, Minnesota Statutes, The Measure of Assertive Community Treatment Fidelity (TMACT) and other Assertive Community Treatment evidence-based literature.
- Must be available to provide overall clinical oversight to the ACT team after regular business hours and on weekends and holidays. The team leader may delegate this duty to another qualified member of the ACT Team, when necessary.
- Maintains optimal program staffing level; hire, oversee staff training.
- Conduct performance evaluations for program staff, address performance issues.
- Assess, refer, coordinate, monitor services and communicate regularly with all providers involved to ensure appropriate care, reduce duplication of services, and promote client autonomy.
- Maintain current knowledge of the mental health service system with the ability to locate information and resources for a variety of client needs, including medical, housing, employment, social services, and community programs.
- Assures financial viability of program.
- Project planning and operations sufficient to negotiate for all human, financial, and technology resources needed for successful completion of the program.
