POSITION SUMMARY: Provides a variety of support services to at-risk suicidal Veterans affected by mental illness. Assists in coordinating services. Provides financial assistance for eligible persons. Identifies and accesses resources. Seeks to expand program and resources independently and confidently. Promotes and maintains public relations. Completes forms accurately and timely. Provides case management, and maintains case fi and case notes. Assists in recruiting and enrolling Veterans into the SSG Fox SPGP. Assists Peer Support Specialist with setting up and running peer support groups. Build relationships with suicide prevention teams at local Veteran Affairs offices.
ROLE AND RESPONSIBILITIES
- Responsible for completing the agency intake form and inputting it into Clarity
- Provide ongoing support to veterans and document via home visits, phone calls, and written correspondence with goal attainment of 65% or above
- Maintain a non-judgmental attitude toward clients and other CNCAP staff
- Ensure clients are assessed through the SSG Fox SPGP process to determine possible assistance through other resources
- Assist Peer Support Specialist in implementing Service Plans with Veteran and/or household members, moving them toward self-sufficiency
- Ensure files are complete with the necessary paperwork for veteran families participating in the program
- Adhere to all CNCAP-related policies and procedures
- Promote the program and agency in a positive and professional manner
QUALIFICATIONS
- High school diploma or GED required (Bachelor's degree in human services or related field preferred)
- Valid driver's license
- Must be a Veteran with lived experience
- 1-3 years of experience in human services or a related field

SCHEDULE: Monday-Thursday: 7 am - 5 pm or Flexible
REPORTS TO: SSG Fox Supervisor
JOB STATUS: Full-Time, 40 hours per week
PAY: $19.50 - $20.50 (depending on qualifications)
