We are seeking a dedicated and compassionate Licensed Master Social Worker to join our multidisciplinary team at the Central New York Psychiatric Center.
About the Role
As a Licensed Master Social Worker, you will be responsible for providing culturally competent and evidence-based treatment and support services to individuals with mental illness. Your duties will include:
- Providing crisis intervention, service linkages, and discharge planning
- Developing treatment plans with individuals and treatment teams, monitoring and evaluating progress, and modifying treatment plans as needed
- Determining discharge needs and appropriate community resources
- Establishing and maintaining relationships with community providers to ensure continuity of care
Responsibilities
Your responsibilities will also include:
- Collaborating with treatment teams to develop and implement individualized treatment plans
- Providing education and support to individuals and their families
- Linking individuals with community resources and services
- Participating in quality improvement initiatives
Requirements
To be considered for this role, you must have:
- A License and current registration in New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
