Under the supervision of the Community Habilitation Program Director the Community Habilitation (CH) Team Mentor provides support in a community setting that promotes success at the highest level of independence possible. Furthermore, the CH Team Mentor will assist the Community Habilitation Program Director in the day to day operations of the Community Habilitation Program.
Requirements:Responsibilities will include but are not limited to the following:
- Maintain a caseload of 15-20 hours of direct Community Habilitation services and assist with intermittent coverage of additional cases as needed.
- Follow all of the responsibilities of a Community Habilitation Specialist as outlined in the job description.
- Conduct Community Habilitation intakes and meet & greets.
- Attend Life Plan and treatment team meetings as needed.
- Coordinate and facilitate the Community Habilitation social groups and specialized activities.
- Complete staff observations and integrity checks.
- Shadow & train new staff with their clients.
- Oversee staff schedules: assign hours, make edits to schedules as needed
- Complete compliance audits & documentation
- Oversee staff training as needed: new employee orientation, DSP core competency, quarterly meetings, annual trainings, etc.
- Meet with DSP’s (Direct Support Professionals) for scheduled supervision (at least monthly)
- Communicate on a regular basis with each Manager of Service and individual served to create an efficient and effective program.
- Meet at least monthly with the Community Habilitation Program Director
- Ensure adherence within the programs to all applicable OPWDD regulations.
- Act in a manner consistent with Central New York Quest’s Mission.
- Other duties as assigned by the Director of Programs.
Requirements:
- At least 2 years' experience as a Direct Support Professional (DSP) working directly with individuals with intellectual & developmental disabilities.
- At least 1 year of supervisory experience
- Valid Driver's License
- Reliable transportation & the ability to travel as needed to surrounding counties for client meetings.
- High School diploma
Full Time: 35 hrs per week
Pay Rate: $20 per hr.
This is an in office work location in Utica, NY and requires travel to the Utica-Rome and surrounding area. Remote working may be an option after 90 days.
Salary : $20
