What are the responsibilities and job description for the Care Coordination Assistant position at Central Ohio Area Agency on Aging?
In this non-clinical role, qualified candidates will field calls for both new and existing clients inquiring about available resources or request status. They will also assist Licensed Social Workers with client recommendations and follows up.
Some individuals in this role may also complete client assessments which may require home visits with $.70 per mile mileage reimbursement.
Required Minimum Qualifications:
- Must have a Valid Motor Vehicle Operator’s license.
AND
- Possession of a bachelor’s degree in one of the social or health care sciences
OR
- A minimum of two years of experience assisting individuals with accessing healthcare, social, or personal services may be substituted for the educational requirement on a year-for-year basis.
Preferred Qualifications:
- A minimum of at least one year of customer service experience
- Experience working with Microsoft Excel
