Central Ohio Geriatrics is expanding in the Granville area. We are looking for LPN’s to work as Care Coordinators with our team in the medical offices.
Central Ohio Geriatrics and our team of Physicians, Nurse Practitioners and Physician Assistants see patients with Care Coordinators at different skilled, long term, assisted living, and independent living facilities providing care for patients in these facilities.
You will be an integral part of coordinating the medical care for our patients in these facilities. The position requires the Care Coordinator to pull together what is medically going on with the patients, and to round with the Physicians, Nurse Practitioners and Physician Assistants to share the information, capture and transcribe into our EMR. We need your keen ability to know our patients and their medical needs.
Responsibilities:
- Facilitate communication between physicians, nurse practitioner, physician assistants, nursing facilities, patients, and patients' families.
- Coordinate and organize patient care census.
- Provide pre-rounding coordination of pertinent wound care data for physicians and advanced practice nurses.
- Duties will be performed in a facility where a patient resides or in a medical office.
- Scribing patient encounters on behalf of physicians, nurse practitioner and physician assistants as directed.
- Record and document physician, nurse practitioner and physician assistant visits on a patient roster to ensure consistent and quality care.
- Responsible for sending progress notes to the provider in an orderly and organized manner.
- Accurately bill patient visits in a timely manner.
- Knowledge of Excel and experience in EMR.
- Excellent organization and time management skills.

- High comfort level working independently.
- Willing to listen and adjust behaviors based on physician/nurse practitioner/physician assistant feedback.
