Job Title: Bilingual Community Health Worker
Reports to: One Community Health Agency Supervisor and Bridges to Health Supervisor
Employer: Central Oregon Independent Practice Association in partnership with One Community Health and Columbia Gorge Health Council's Bridges to Health program.

Location: Primary location in Hood River with ability to drive to The Dalles on a weekly basis to meet with clients and coordinate with co-workers.
Purpose of the Position: The Community Health Worker will be responsible for helping community members and households navigate and access community services, health services, other resources that promote healthy behaviors. The Community Health Worker works closely within their own site organizations team as well as alongside other Community Health Workers from organizations both in and out of the Bridges to Health Pathways Program.
A typical day for a Bridges to Health Community Health Worker will entail spending half the day conducting one on one extended client and/or family visits (in office and out in the community). The other half the day will be spent on follow up with clients, providers and resources to ensure referrals have been made successfully as well as within the home organization to respond to new clients that might arise.
Summary of Essential Job Functions:
Client Engagement and Services:
- Build relationships and engage all clients/families through culturally and linguistically appropriate care coordination
- Complete Enrollments and Assessments on all clients and enter into data system
- Provide care coordination, ensuring follow up and referrals for resources are completed
- Work alongside clients/families to increase their ability for self-management and shared decision-making
- Identify needs, gaps and duplicate services and collaboratively find solutions with other agencies to create efficient client support services
- Advocate on behalf of the needs of the client to successfully navigate needed services
- Ensure documentation and effective communication when service gaps are found
- Utilize and build on existing local resources
Community Collaboration and Skill building:
- Engage and strengthen relationships with other Community Health Workers and community partners within the Bridges to Health program
- Cultivate and support relationship building with health care, dental care and social service providers
- Attend all Community Health Worker training courses/ webinars and required meetings
- Provide feedback for the improvement of the Bridges to Health Pathways Program
Competencies and Skill Requirements:
- Successful completion of a Community Health Worker formal training program and Oregon State certification is preferred. Otherwise, you will be required to complete a CHW certification training within one year. Cost will be covered by employer.
- Direct person care job experience, training, certifications or licenses will also be considered (CNA, HHA, CMA, Caregiving, Direct Support Professional, etc...)
- Lived experience preferred
- Bilingual Spanish/English required
- Commitment to providing culturally and linguistically appropriate care coordination with client and family- centered approach
- Demonstrate professional, appropriate, effective, and tactful communication skills, including written, verbal and nonverbal
- Has an ability to adequately capture all required documentation in web based data systems within the timeframe required per the program
- Demonstrate familiarity with Microsoft programs
- Demonstrate a positive attitude and respectful, professional customer service
- Acknowledge client’s rights on confidentiality issues, maintain client confidentiality at all times, and follow HIPAA guidelines and regulations
- Proactively act as client advocate, responding with empathy and respect to resolve client and family concerns, and recognize opportunities for improvement to meeting client concerns
- Proactively continue to educate self on providing quality care and improving professional skills
- Ability to travel within the Gorge is required; valid driver’s license and insured vehicle
Salary:
$20.85 – $26.18 hours depending on experience. Top out pay up to $36.50. Bilingual differential of $2/hour
