What you bring to SBHG:
- Master's degree in psychology, social work, counseling, or other mental-health related field required.
OR
- Graduation from an accredited school of licensed vocational nursing or psychiatric technician required.
- (2) years direct service experience with severely emotionally disturbed adolescents, in an inpatient health care setting preferred.
OR
- (1) year service experience medically treating the mentally ill in an inpatient setting preferred.
- Valid California LVN, LPT, AMFT, APCC, ACSW, LMFT, LPCC, or LCSW required.
How you will make a difference:
- Makes referrals and help parents and other people who call needing help with mentally ill relatives or clients when hospitalization at the PHF is not indicated.
- Establishes and maintains an on-going relationship with county agencies, health care facilities, insurers, and other resources necessary to affect the continuum of care for clients returning to the community.
- Assures all legal admission requirements are obtained for each new client including involuntary 5150, voluntary by youth (Roger S), conservatorship, or clients signed in by parents/legal guardians.
- Assists with intake assessments of new admissions during periods of unusual unit activity (crisis situations, staffing shortages, etc.).
- Participates actively in assigned meetings including treatment team meetings when needed.
- Upon admission, enrolls the client in the EMR or provides the appropriate information for Nursing or PHF Clerk/Reception staff.
- Takes photo of client and upload to EMR (or provide to Nurse or PHF Clerk/Reception to upload to EMR chart).
Division/Program Overview:
- 24/7 inpatient involuntary program
- 16-bed facility for acute youth (ages 5-17)
- Intensive treatment programs with individualized care plans
- Available for contracting Central Valley counties
- Moves clients through five phases: admission; engagement; learning; stabilization and linkage to resources and aftercare
