SUMMARY- This position is first shift (day shift) only.
Responsible for all clinical activities, coordinates activities of the home managers, and oversees the homes by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES include but are not limited to the following. Other duties may be assigned.

Accountability:
The Home Supervisor answers directly to the Program Coordinator. He/She is responsible for completing all routine and assigned duties related to consumer care and home operations. Additionally, he/she is responsible for all other tasks, duties and responsibilities assigned or delegated by the Program Coordinator, and/or Executive Director.
Home Supervisor General Responsibilities:
1. Assist or provide consumers with all aspects of personal care.
2. Assisting consumers by lifting, transferring, and when necessary, to prevent injury, physically managing them.
3. Accompany, assist and supervise consumers in the Community, and to events. Encourage appropriate, integrated social and recreational community interaction.
4. Administer and document medications as prescribed by the Physician. Record errors on an Incident Report form or a medication error report form and report to the Program Coordinator immediately.
5. Assure active treatment and implementation of Person Centered Plans.
6. Provide constant supervision of all consumers to assure that they are safe and do not wander from the home or yard.
7. Observe, document and report unusual behavior and/or incidents.
8. Maintain records and documentation as necessary and appropriate.
9. Perform any and all other duties, tasks and responsibilities assigned by Program Coordinator and/or Executive Director.
SUPERVISORY RESPONSIBILITIES
Directly supervises two to thirty employees at the assigned work site. He/She will carry out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include:
- conducting in home training, scheduling and coordinating staff in-services
- planning, assigning, and directing the work of at the work site [scheduling staff hours]
- evaluating staff performance on a regular and scheduled basis
- supports and strengthens the corporate character program
- recognizing and rewarding the demonstration of character by employees, people receiving supports and other members of the community
- correcting performance of employees that does not meet position expectations
- addressing complaints and resolving problems
- conducting monthly staff meetings and other meetings as they become necessary
- monitoring all documentation, progress notes, medications, incident reports, emergency drills and procedures, telephone logs, staff communication logs, and flow charts to assure routine tasks have been completed
- completing time sheets and faxing them to the office on the first business day following the end of each pay period by noon on Monday
- keeping accounts for the petty cash and mailing all receipts to the office for reimbursement
- keeping strict accounting of the funds of the person receiving services
- ensuring that food, housekeeping supplies and medical supplies are ordered, purchased and received
- securing approval of a plan for any needed maintenance repairs
- scheduling and attending all medical appointments
- acting as a liaison to all agencies, to the Case Manager and to all family members involved in services for the person
Accountability:
The Home Supervisor answers directly to the Program Coordinator/Executive Director. The Home Supervisor is responsible for all aspects of consumer care, staff supervision and home operations addressed in the Home Supervisor's Manual and for all other duties, assigned or delegated, by the Program Coordinator/Executive Director. Additionally, the Home Supervisor is responsible for meeting the requirements and standards of the Michigan Department of Human Services, the Michigan Department of Community Health, Community Mental Health Boards, CARF and when applicable, Special Certification.
General Responsibilities:
1. Assure that Person Centered Plans are followed, monitored and documented, as necessary.
2. Monitor all documentation and required record keeping. Provide training to staff and follow-up administratively if there are omissions or errors.
3. Review all incident reports and investigative findings (administrative reports) and take appropriate action, processing them immediately.
4. Assure that all procedures and documentation related to medications are completed correctly. Provide training to staff and follow up administratively if there are omissions or errors.
5. Maintain the home, van, grounds, furnishings and equipment according to agency and licensing standards.
6. Establish/maintain good public relations in the neighborhood and the community at large.
7. Respect and assure the confidentiality of all consumers and staff by protecting and keeping private information and records.
8. Establish and maintain good relationships with support staff and DHS/DCH/CMH administrators.
9. Maintain a schedule appropriate to the needs of the consumers and the provisions of the home's contract.
10. Make appointments for medical, dental, lab work, haircuts, consultations, etc. for consumers.
11. Schedule and chair staff meetings monthly and document and file. Conduct required monthly in-services.
12. Assure compliance with emergency procedures and requirements for fire and tornado drills, as outlined in the emergency preparedness manual
13. Train, evaluate, supervise and discipline staff.
14. Coordinate the hiring and dismissal of staff with the Program Coordinator.
15. Maintain personnel files and consumer records per Central State Community Services, Inc., Michigan DCH/CMH and DHS standards.
16. Conduct and participate in training's, ensuring the completion of 16 hours of training per licensing standards.
17. Act as a liaison for Central State Community Services, Inc., to all agencies and individuals involved in the lives of consumers.
18. Assure that menus, grocery shopping lists, food purchasing procedures and dietary record keeping meet all applicable standards, and approved substitutions.
19. Provide 24 hour on call emergency availability to home staff (coordinate with Assistant Home Supervisor and Program Coordinator).
20. All other tasks, duties and responsibilities as assigned by the Program Coordinator or Executive Director.
21. Medical Coordinator Responsibilities
22. Write out and update medication records, according the CMH transcription's guidelines. The consumers need to be informed of all their medications. All medications needs to be kept locked or in a box with a lock.
23. Responsible for seeing that medication count is done daily.
24. Prepare lab work and make sure that the consumers are taken to their scheduled lab draws.
25. Coordinates the consumer's injections with home nurse.
26. Attends all med reviews and expresses any concerns about the consumer. A med review book is taken and all information is documented in each of their sections. Appointment record form is documented on each appointment and filed in the record
27. Schedule all routine doctor appointments and make sure a request (A paper for dental, surgical and medical services that needs to be confirmed by your case manager for payment) is filled out for every doctor appointment.
28. Schedule an appointment and fill out a consult (A paper given to the doctor explaining why the patient is being seen and what medications they are on, the doctor will fill it out with his/her outcome) that will be given to your home nurse. The medical appointments or any health problems need to be documented in the HCC book, homecare staff notes, staff log and informed to your home nurse, case manager (if needed), home supervisor, staff, and last but not least, the consumer.
29. Medical appointments need to be informed with your home supervisor, the consumer and if needed the staff.
30. Medical appointment cancellations need to be completed 24 hours prior to the appointment.
31. Updated scripts, new scripts and discontinued scripts need to be faxed over to your home nurse and your pharmacy and then transcribe all the information in the HCC book, med book, homecare staff notes, and staff log (if county requires). Transcribe by copying information from the pharmacy label to the medication sheet. The self-med consumers need to transcribe their meds as it says on the pharmacy label with some assistance from the med coordinator and then revised for any errors. Scripts need to be noted with start date and initials.
32. Sets up leave of absence (LOA) medications. For each medication, a LOA bottle is needed.
33. Standing Medical Orders (SMO's) need to be kept locked up and updated.
34. Oral and Topical medications need to be kept separate in med cabinet and also locked up
35. Making sure the staff is following all medication procedures deemed necessary by Specialized Residential Services.
36. Insurance cards are kept in their med book.

QUALIFICATIONS

- Copy of valid and unrestricted Michigan Driver's License.
- Minimum of a high school diploma or equivalent.
- Three or more positive references.
- Evidence of ability to handle crisis situations with calmness and objectivity.
- Ability to maintain and record expenditures as well as accounting for and safeguarding funds, valuables and confidential materials.
- Complete and meet the requirements of a criminal history check.
EDUCATION and/or EXPERIENCE
High School diploma or (GED).
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, physician’s orders, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to effectively present information in one-on-one and small group situations to individuals and other employees of the organization.
MATHEMATICAL SKILLS
Ability to add and subtract two digit numbers and to multiple and divide with 10’s and 100’s. Ability to perform these operations using units of American money and weight measurements, volume and distance.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS
A valid Michigan driver’s license is required as well as current insurance as required by the State of Michigan. Group Home curriculum training must be successfully completed within the first 90 days of employment.
OTHER QUALIFICATIONS
A felony/misdemeanor background search with MILT and IChat is performed. Searches of the Office of Investigator General (OIG), Offender Tracking Information System (OTIS), the Sex Offender Registry and the Nurse Aid Registry are also performed. Results can affect employment status.
