What are the responsibilities and job description for the PACE Home Care Clinical Supervisor RN (Central Valley PACE - Modesto) position at Central Valley PACE?
Under the direct supervision of the PACE Clinical Manager RN, the Home Care Clinical Supervisor RN is responsible for the development and implementation of homecare nursing home/boarding care services for program participants and oversight of all clinical skilled home care services.
Schedule is Monday – Friday, 8:00am – 5:00pm
Compensation: $52.42 - $60.68 an hour

Duties and Responsibilities
- Assesses, using the nursing process, the home care needs of a frail elderly population, and identifies and develops specific plans of care. Initial and periodic assessments to be conducted minimally every six (6) months and must be completed prior to the scheduled team meeting.
- Responsible for the provision of home care to meet identified participant outcomes, and to achieve the goals as outlined in the participants Plan of Care
- Communicates participant changes with team members, coordinates and authorizes all Durable Medical Equipment.
- Coordinates and supervises care plans for participant that are residing in the nursing home/boarding home. Includes interaction with other facility staff.
- Supervise the delivery of skilled services and personal care services in the participant’s home of our staff and quality of contracted services in the home.
- Performs evaluations of HCT's, and resource management of HCT staff in conjunction with Center Manager.
- Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members.
- Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
- Ensures all regulations and standards are up to date, keeping compliance with orders or directives issued by constituted government/regulatory agencies. Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
- Other projects and duties as assigned.
Physical Demands
- Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.
- Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
- Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
- Must have high manual dexterity.
- Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
Work Environment
- Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
- Subject to unpleasant odors.
- The noise level is usually quiet to moderate, but may at times be noisy and crowded.
Minimum Qualifications
- Valid CA Driver’s License, acceptable driving record, and vehicle insurance.
- Must be able to work effectively in a team environment. Must be able to treat geriatric patients.
- Only act within the scope of his or her authority to practice.
- Ability to communicate skillfully, handle multiple priorities, maintain a warm and friendly demeanor, perform nursing duties, be flexible, innovative, focus on results and travel regularly.
- Meet a standardized set of competencies established by PACE and approved by CMS before working independently.
Education/Experience
- High school diploma or equivalent. Graduate of an accredited school of professional registered nursing.
- Current unencumbered CA Registered Nurse (RN) License.
- Current BLS CPR Card certified by American Heart Association.
- Minimum one (1) year of experience in a home health agency, primary care clinic, or health facility as a registered nurse or license vocational nurse; or minimum five (5) years as a CNA or MA.
- Minimum one (1) year as in a supervisory or administrative capacity.
- Minimum one (1) year working with the frail or elderly population.
