- Have at least 1 year of professional-level experience in human relations or related field is preferred

The Role:
Provides non-clinical services in assisting families by monitoring/following up, linking, referral, advocacy, and crisis intervention.
What you will do:
- Conducts thorough and comprehensive case management assessments that identify the individual strengths and needs of each client
- Synthesizes assessment information to prioritize problems that can be addressed by available services and resources
- Coordinates and collaborates with clients and the therapist to develop personalized, achievable goals, objectives and interventions that promotes self-reliance
- May perform other related duties as assigned
Qualifications:
- Bachelor's degree in human relations field required
- Minimum of 1 year's professional-level experience in human relations or related field is preferred
- Completion of a pre-employment drug screen
- Completion of a background check
- Satisfactory Driving Record
