Diversion and Re-Entry Specialist

The Role:

The Diversion and Re-entry Specialist plays a crucial role in facilitating communication and coordination between various entities involved in the clinical aspect of the legal process to facilitate diversion for people in need of mental health and substance use disorders services. 

What you will do:

• Provide supportive pre-assessment engagement activities, through phone calls or in person, including problem-solving, patient education, and referral to resources
• Deliver regular, individualized personal assistant to clients who are transitioning levels of care when at risk for disengagement
• Accurately and completely document all patient activity in complete notes prepared by close of business on the day of contact and ensure client files are maintained in accordance with CWCC's policies and procedures
• Attend community meetings with partnering agencies regarding procedures for accessing services specifically related to emergency situations
• Attend meetings as notified
• Perform other administrative tasks
• May perform other related duties as assigned

Qualifications:

• Minimum of 1 years' experience is required
• Peer Specialist Certification required
• Previous case management experience is a plus
• Previous experience in justice-involved programs is a plus
• Bachelor's degree in business related field is preferred
• Strong Attention to organization and detail
• Strict ability to maintain confidentiality, and professionalism
• Completion of a pre-employment drug screen
• Completion of a background check
