As a Home Care Nurse, you will play a pivotal role in providing specialized nursing care to children and adults with complex medical needs within the comfort of their own homes. Responsibilities:
- Provide skilled nursing care to our teenage client, including:
- Tracheostomy care and management
- Ventilator management
- G-tube feeding administration
- Medication administration
- Monitoring vital signs and overall health status
- Providing emotional support and companionship
- Collaborate with the client’s family and interdisciplinary team to ensure comprehensive care.
- Maintain accurate and timely documentation of care provided.
- Thursday, Friday, Saturday, and Sunday
- 7:00 AM - 3:00 PM
- Valid RN or LPN license in the state of Michigan.
- Current CPR certification.
- Experience with trach, vent, and g-tube care preferred, but we are willing to provide onsite training for the right candidate!

Responsibilities
- Perform duties outlined in patient's plan of care
- TBI SCI rehabilitation
- Pediatrics & Adults
- Ventilator and Tracheotomy care (Training Provided)
- Gastrostomy tube care (Training Provided)
- Dispensing medications
- Observe client’s physical, mental and emotional conditions.
- Report any changes
- Document observed changes
- Communicates effectively with Clinical team to increase optimal outcomes
- Maintains professional nature within the home setting
- Excellent oral and written communication skills
- Ability to follow verbal and written instructions
- Ability to utilize computers/tablets and computer software
- Current state license as an RN or LPN
- Valid driver’s license and reliable transportation
- Current negative TB skin test or negative chest X-ray
- Current flu vaccine
- CPR certified
