The client is a 23-year-old individual currently residing in a hospital bed located in the living room due to limited mobility. He requires comprehensive care, including tracheostomy and gastrostomy (G-tube) management.
He has a diagnosis of Lennox-Gastaut Syndrome (LGS), a severe and rare form of epilepsy that typically presents in early childhood. This condition is characterized by multiple types of seizures, including atonic, tonic, and atypical absence seizures. Due to the complexity of his medical needs and seizure activity, the client would benefit significantly from Private Duty Nursing (PDN) services to ensure his safety, support ongoing medical care, and improve his quality of life.

Responsibilities:
- Provide skilled nursing care, including but not limited to tracheostomy care, ventilator management (if applicable), catheter care, wound care, medication administration, and bowel/bladder programs.
- Assist with activities of daily living (ADLs) such as repositioning, transfers, and hygiene.
- Monitor the patient’s vital signs, symptoms, and overall well-being, reporting any concerns to the healthcare team.
- Offer emotional support and companionship, ensuring a positive and comfortable environment.
- Collaborate with family members, caregivers, and medical professionals to maintain continuity of care.
- Licensed RN or LPN in the state of Michigan.
- Experience in home health, private duty nursing, or long-term care preferred.
- Knowledge of paraplegia-related care, including skin integrity, mobility assistance, and respiratory support, is a plus.
- Ability to provide compassionate and patient-centered care.
- Strong communication and teamwork skills.
- Full-time, part-time, and per diem shifts available (Day, Night, and Weekend shifts).

Responsibilities
- Perform duties outlined in patient's plan of care
- TBI SCI rehabilitation
- Adult/ Pediatrics
- Ventilator and Tracheotomy care (Training Provided)
- Gastrostomy tube care (Training Provided)
- Dispensing medications
- Observe client’s physical, mental and emotional conditions.
- Report any changes
- Document observed changes
- Communicates effectively with Clinical team to increase optimal outcomes
- Maintains professional nature within the home setting
- Excellent oral and written communication skills
- Ability to follow verbal and written instructions
- Ability to utilize computers/tablets and computer software
- Current state license as an RN or LPN
- Valid driver’s license and reliable transportation
- Current negative TB skin test or negative chest X-ray
