What are the responsibilities and job description for the Hospice RN Case Manager position at Centric Home Health and Hospice?
We are currently hiring for a Full-Time RN Hospice Case Manager.
The Hospice RN Case Manager is responsible for coordination and delivery of skilled nursing care to an assigned group of patients. Most of the work is performed in the patients’ residences or nursing facilities.
Responsibilities include but not limited to:
- Conducts the admission of the patient to the Agency
- Makes sure clients meet hospice criteria
- Coordinates and provides skilled nursing care as outlined in the nursing care plan
- Performs supervision of clinical staff, nurses, and Certified Nursing Aides providing care to our clients
- Attends IDG Meetings
- Takes On-Call
Qualifications
- Graduate of an accredited school of nursing.
- Licensed by the Oklahoma State Board of Nurses.
- Minimum two (2) years experience as a registered nurse in clinical practice.
- At least one (1) year of hospice experience.
- Prefer candidates with one (1) year experience as a RN Case Manager.

Pay: $75,000.00 - $90,000.00 per year
