The Community Health Worker provides education and navigation to clients who have been impacted by COVID-19 and MPOX, and also works with foster families under our KEEP Program.
PRIMARY RESPONSIBILITIES
- Provides education to clients to understand preventive diseases, specifically about viruses such as COVID-19 and Mpox.
- Assist clients by tracking case management.
- Provides navigation to get access to vaccines.
- Collaborate with clients to develop an action plan to address health needs and goals.
- Serve clients under isolation/quarantine as needed for "case management" during the period in which the person is in isolation/quarantine.
- "Case management" activities include providing a list of clinics to get vaccines, scheduling appointments, doing referrals, and explaining invoices or bills.
- Handle confidential information with discretion and professionalism and in compliance
- Explore alternate resources and connect clients to community services
- Monitors progress and needs and makes necessary modifications to the case plan.
- Manage a high volume of calls or interviews with individual contacts
- Obtain documentation, collect required data and complete the intake paperwork for eligible clients.
- Documents and keeps files of all client services, including date, referrals to other organizations or agencies. Maintain virtual files updated; through case notes.
- Help with other team projects and attend meetings as assigned.
- Developed workshops and materials, under the approval of the supervisor.
- Attend training and meetings in person or virtual.
- No responsibility for directing or supervising others.
- Providing general information, application forms, and declaration forms.
- Schedules clients in need of immediate services to appropriate resources.
- Answer a variety of questions related to the department programs process.
- Communicate effectively verbally and in writing with individuals from diverse socio-economic and cultural backgrounds.
- Follow written and oral directions and instructions.
- Other duties as assigned

- CHW Certificate preferred

- Strong mathematical skills
- Working knowledge of Microsoft, Google Drive, Salesforce
- Fluent Spanish language speaker preferred

- Demonstrates customer service skills and proficiency with G-suite products.
- Learn appropriate questioning content and techniques for interviewing applicants.
- Enter data accurately into an automated system.
- Community Health Worker certification, preferred
- Bilingual Spanish-English, with the ability to translate documents, agendas, and other materials as needed
- Maintain and adhere to all Federal, State, and County regulations regarding client confidentiality, specifically 45 CFR-Part 160-164, HIPAA.
PHYSICAL DEMANDS/WORK ENVIRONMENT
This position is approximately 50% in an office environment and 50% in community locations. Office working conditions are standard for an office environment. While performing the duties of this job, the employee will:
- Engage in regular communication in person as well as through electronic means.
- Routinely use computer laptops, tablets, desk phones, photocopiers, and monitors, including digital screens.
- Frequently move up to 30 pounds.
