What are the responsibilities and job description for the Social Worker position at Centro de Salud Familiar La Fe?
Responsible for providing culturally relevant comprehensive social services to clients in a medical clinic setting. Services include assessment, appropriate intervention and case management, follow up, advocacy and information and referral to community resources. Essential part of social services include accurate documentation of services provided.
Responsibilities and Duties
- Responsible for providing courteous assistance to all individuals entering the clinic.
- Responsible for the provision of quality social services.
- Responsible for maintaining accurate and current record keeping functions.
- Responsible for the compilation, preparation and presentation of reports.
- Attends and participate in regular Board committee as well as other meetings,conferences, workshops, governmental organizations, etc. as assigned.
- Establishes and maintains positive interpersonal relationships and keeps line of communication open with treatment team and community agencies.
- Responsible for referring patients to relevant and appropriate community resources,social service agencies, medical, and/or emergency assistances agencies.
- Performs other duties as assigned.

- Ability to conduct Bilingual/Bicultural interviews and complete appropriate case documentation.
- Ability to provide crisis, intervention, casework and case management, assessment, planning, implementation, monitoring and evaluation of psychosocial and financial needs of clients.
- Ability to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served.
- Knowledge of the principles of growth and development over the life span.
- Ability to identify and categorize each patient’s age-specific grouping of needs, such as those for infant, adolescent or aging patients.
- Bilingual English/Spanish.
Required Education
- Bachelors degree in Social Work.
- One year training in Social Services field.
Required Experience
- One year experience working as a caseworker in a health, mental/behavioral health or related setting.
License/Registration/Certification
Bachelor’s in Social Work.
