THE PROGRAM
Centro La Familia Advocacy Services (CLFA) is implementing the La Puerta Hacia Las Oportunidades – Door of Opportunities project with partner Fresno Area Workforce Investment Corporation. This project focuses on providing Integrated Education and Training (IET) to the English Language Learner (ELL) populations in Fresno, Madera, Kings, and Tulare Counties. CLFA is seeking a Case Manager/Navigator to carryout services. The Case Manager/Navigator will recruit clients for IET services through various outreach methods and link them to Fresno Area Workforce Investment Corporation for employment and training services. Case Manager/Navigator will track referred clients and provide case management, linkages to other services, and wraparound support. They will report to the Program Supervisor. This project is funded by the State of California’s Employment Development Department.
BASIC FUNCTIONS
The Case Manager/Navigator will work within its staffing team and attend outreach events, resource fairs, presentations, and other local venues to promote IET services. They will inform the public of the available employment and training services and encourage them to participate. Target population is English Language Learners in Fresno, Madera, Kings, and Tulare Counties. Once clients are identified and referred, they will be engaged in CLFA’s case management services and linked to additional services that help meet their needs. Clients will receive a thorough assessment process and supported with a case plan, and goal setting. Case Manager/Navigator will also track deliverables, meet reporting requirements, attend meetings and trainings, and work closely with their Program Supervisor.
JOB RESPONSIBILITIES
· Implement intake and assessments to identified clients following CLFA’s established intake process.
· Conduct intensive case management services including completing forms, case notes, and more.
· Follow the identified SOW activities and meet program deliverables including referrals to IET services, linkages, and others.
· Follow CLFA’s comprehensive case management processes including tracking, case notes, and follow ups.
· Monitor clients progress determining access barriers and eliminating them.
· Interpret rules and regulations on various topics for clients.
· Follow all confidentially processes to protect client’s information.
· Attend outreach events in urban and rural communities with the goal of promoting IET and CLFA services.
· Connect community members with resources providing linkages and system navigation.
· Meet reporting requirements including the Monthly Activity Report, program reports, and others.
· Reach populations through outreach events, social media, media, presentations, and other venues.
· Work closely with the Fresno Area Workforce Investment Corporation and other partners.
· Participate in weekly and monthly meetings and trainings in person or virtually.
· Performs other related duties as assigned.
· Ability to work some evenings and weekends.
· Associate’s or Bachelor’s degree in public health, health science, or related field
· Experience working full time as a Peer Support Specialist, Health Educator, Navigation Specialist, or similar field.
· Understanding of the Promotora model
· Excellent interpersonal skills to assist distressed individuals from different cultural backgrounds.
· Experience working in community outreach, and/or education programs preferred.
· Bilingual a plus, preferably in English/Spanish
· Proficiency in MS Word, Excel, and Outlook Express, as well as data entry
· Ability to comply with regulations and communicate official information in an appropriate manner.
· Advanced ability to coordinate efforts, facilitate intervention, and maintain records.
· Exceptional organizational skills and sound judgment
· Valid CA driver’s license and auto insurance
· Satisfactorily complete comprehensive training delivered by funder and other partners during onboarding and will be responsible for assisting in the training of staff and volunteers as needed.
· Attend mandatory training as part of their orientation and onboarding and will comply with state and federal regulatory requirements related to protected health information and privacy.

TYPICAL WORKING CONDITIONS: The office setting is a normal environment. Occasionally work during early morning, evening, or weekend. May me subject to temperature variances outdoor during events.
TYPICAL PHYSICAL DEMANDS: Requires sitting, standing, or walking for up to eight hours a day. Some bending, stretching, or reaching may be necessary. Lifting to 40 pounds may be required on occasion. Vision must be correctable to 20/20 and hearing must be in the normal range for telephone contact.
