THE PROGRAM
Centro La Familia Advocacy Services (CLFA) is seeking a qualified candidate to oversee and implement services for its California Advancing and Innovating Medi-Cal (CalAIM) Program. This program is a reimbursement based program that is made viable on provides eligible families in Fresno County with Community Supports (CS) in the areas of housing navigation, housing deposit, and housing tenancy services. The goal is to reduce homelessness and achieve housing stability for qualified families who are CalViva eligible and active. The Program Manager will oversee and implement services that will help families meet housing goals including an intake and housing assessment, building a housing case plan, budgeting, goal setting, linkage to services, reporting and others. All services will be tailored to meet a family’s individual needs and services will be provided according to the principles of Housing First. The CalAIM program is in collaboration with CalViva Health/Health Net.
BASIC FUNCTIONS
The Program Manager (PM) will oversee staff and cohesively carryout services in the areas of housing navigation, housing deposit, and housing tenancy. They will ensure staff establish trust with families and identify challenges, barriers, and needs families have. The PM will oversee direct services and maintain a caseload. Direct services include screening clients for eligibility, implementing intake and assessments, developing housing plans, implementing housing navigation, providing housing deposits, and housing tenancy in the forms of linkages, and follow ups, and other duties. The PM will attend meetings, trainings, complete reports, and track financial information related to program services. They are responsible for staff oversight, caseload management, scheduling, and tracking. They will work closely with the Executive Director and the Fiscal Department.
QUALIFICATIONS
Candidate must possess excellent written and verbal communication, ability to follow-through, and be detail oriented. Candidate must be organized, professional, and able to work in a fast-paced, multi-faceted environment. Candidate must be proficient in the use of technology to collect, analyze and manage program data as well as computer literate in Windows platform, proficient in Microsoft Word, Excel and Google Applications. Candidate must possess the ability to maintain good recordkeeping and documentation, good presentation skills, and have the capacity to be self-motivated. It is mandatory to meet employment eligibility requirements as established by the program’s funding agency.
PRIMARY JOB RESPONSIBILITIES
· Responsible for the administration, planning, organization, and operation of the program.
· Utilize designated database for services and CLFA’s internal Case Management System.
· Ensure clients are guided and referrals are made for housing navigation, housing deposit, and housing tenancy services.
· Ensure clients are screened through identified portals to ensure eligibility.
· Track billings and reimbursement of client services.
· Directly supervise staff and create benchmarks that are attainable and sustainable.
· Engage with families to assess immediate and long-term housing and develop tailored housing plans with families.
· Provide housing transition services including identifying barriers to housing, search and secure housing, habitable environments, and others.
· Providing housing deposits.
· Provide housing tenancy and sustaining services which includes intervention of jeopardized housing, rights and responsibilities, and others.
· Conduct and oversee landlord engagement.
· Ensure linkages are provided.
· Responsible to track information correctly for the purpose of reports, and billable activities.
· Maintain calendar via outlook tracking appointments, events, meetings, trainings, others.
· Maintain accurate client files, recordkeeping, and adhere to confidentially policies.
· Maintain knowledge of CalViva Health CalAIM rules and regulations.
· Adhere to the Health Information Exchange (HIE) and Health Insurance Portability and Accountability Act (HIPAA) regulations for all provision of services.
· Participate in meetings and trainings in person or virtually.
SECONDARY JOB RESPONSIBILITIES
· Maintain knowledge of community resources including food distributions, transportation, clothing, medical care, employment services, mental health, and others.
· Provide community education by attending outreach events such as vaccination clinics, resource fairs, community forums, presentations, and others.
· Track community education provided including sign in sheets, materials distributed, others.
· Support in recruiting, onboarding, and the training and development of staff.
· Lead, inspire, cultivate, and manage staff.
· Develop a sense of belonging and teamwork among CLFA staff and collaborative partners including landlords to place clients in housing and or prevent homelessness.
· Organized, self-motivated, professional, and ability to work in a fast-paced, multi-faceted environment.
· Proficiency in the use of technology to collect, analyze and manage program data as well as computer literate in Windows platform, Microsoft Word, Excel, and Google Applications.
· Empathetic, identifies and communicates effectively with diverse communities, in a culturally sensitive manner that emphasizes the importance of relationship and trust building between the client and advocate.
· Implement financial education and coaching in efforts to stabilize client’s situation.
· Monitor client’s progress and assist in resolving barriers to services.
· Must have a valid CA driver’s license and proof of insurance.
· CA Department of Justice fingerprint clearance.
· Performs other related duties as assigned.
Preferred Education/Experience
· Bachelor’s from an accredited college with a major in social work, social welfare, human services, or related field
· Minimum of four years’ experience in a community based human service agency as an advocate, navigator or other related work experience.
· Bilingual abilities in another language common to the area is a plus but not required (English/Other language)

TYPICAL WORKING CONDITIONS: The office setting is a normal environment. Occasionally work during early morning, evening, or weekend. May me subject to temperature variances outdoor during events.
TYPICAL PHYSICAL DEMANDS: Requires sitting, standing, or walking for up to eight hours a day. Some bending, stretching, or reaching may be necessary. Lifting to 40 pounds may be required on occasion. Vision must be correctable to 20/20 and hearing must be in the normal range for telephone contact.
