Summary:
The Personal Care Attendant/HHA is responsible for providing personal care, meal preparation, light housekeeping, and errands to persons served in their homes as outlined by each individual’s Care Plan.
Essential Duties and Responsibilities:
· Assist persons served with personal care needs such as bathing, elimination needs, transferring, dressing, undressing, and ambulation together with any household task (e.g., making the bed, changing linens, laundry, light housekeeping, etc.) essential to the patient’s health following the patient’s individualized care plan.
· Document services provided accurately and timely in accordance with agency policy and personal care plan. HHA must always maintain patient confidentiality.
· Report any changes in the physical or emotional condition of the person served or incidents to CENTRO’s GAFC program administration on time.
· Notifies Program Administration when clarification or education of a task is required.
· Submits activity sheets on time (i.e., weekly)
· Demonstrates appropriate judgment skills to make independent decisions in routine patient care matters.
· Assist patient/person served (PROM) when requested and/or directed by the Nursing Supervisor
· Attends and participates in staff meetings, required in-service, agency performance improvement activities, and other meetings as requested.
· Mandated Reporter Statute: According to MA Statutes, an individual in this position is considered a “Mandated Reporter” when on duty.
Other Duties & Responsibilities:
· Performs other duties as assigned by the supervisor.
Education & Qualification Requirements:
- Excellent written and oral communication skills.
- Excellent organizational skills
- Self-motivated
- Resourceful
· Must be at least 18 years of age.
· Personal Caregivers who are certified personal care homemakers or certified home health aides or successfully pass the competency evaluation given by CENTRO’s registered Nurse that determines the direct care aide is competent in all the areas listed in 408.524 C.d.1-11
- Fluency in Spanish preferred.
Physical Demands:
· Ability to be physically mobile (stand, walk, reach, climb, bend) and the ability to sit for prolonged periods throughout the day/work hours.
· Manual dexterity sufficient to operate keyboard, copier, telephone, and the ability to write and/or type.
· Must be able to lift and/or move up to 25 pounds occasionally.
· Occasional driving to person-served appointments, offsite meetings, conferences, etc.
Other Requirements:
· Valid driver’s license
· Recent physical exam and TB screen within 12 months
