What are the responsibilities and job description for the (Licensed Practical Nurse) Temp to Perm 40 Hours position at Centro Las Americas?
Position Summary:
The AFC & GAFC Program Licensed Practical Nurse under the direction and supervision of the AFC & GAFC Registered Nurse, the LPN is responsible for conducting medical assessments of the person served, as well as maintaining communication with the person served medical team. In addition, this person is responsible for identifying and accessing additional medical resources to support and to assist person served and their families in his/her caseload, and for maintaining direct communication with the Registered Nurse Supervisor and Program Director.
Duties & Responsibilities:
Under the direction and supervision of the AFC & GAFC provider RN, licensed practical nurses may:
• Complete and coordinate all applicable clinical assessments and clinical evaluations;
• Develop and review on an ongoing basis each person served AFC plan of care;
• Provide oversight to AFC caregivers in conjunction with the care manager;
• Review the PCP Summary Form;
• Monitor the health status of all person served to ensure that all needed AFC is properly delivered;
• Report changes in the health status of any person served to the member's PCP;
• Review AFC caregiver logs at a minimum of every 30 days;
• Conduct on-site visits with each person served at the qualified setting in accordance with 130 CMR 408.415.
• Complete a progress note corresponding with each on-site visit or encounter, or more often as the person served condition warrants;
• Coordinating the delivery of AFC with any other health services or supportive services the person served is receiving from MassHealth or other agencies or organizations, including but not limited to, visiting nurse services, therapy services, Department of Developmental Services (DDS) services, Department of Mental Health (DMH) services, and Massachusetts Rehabilitation Commission (MRC) services;
• Submit semiannual health status reports to the person served PCP;
• Plan for and implement discharges and transition from the AFC program;
• Provide ongoing training to AFC caregivers on health and aging.
• Coordinating and implementing the PCP form and approval for AFC with the person served, AFC caregiver, and AFC provider personnel;
• Developing, in conjunction with the MDT the AFC caregiver, and the person served or responsible party, an emergency backup and personal care contingency plan for each person served receiving AFC & GAFC that includes an alternative plan for the person served if the AFC caregiver is temporarily unable to provide care; and
• Overseeing, monitoring, supporting, training, and evaluating AFC caregivers.

Education & Experience:
• The licensed practical nurse must be licensed by the Massachusetts Board of Registration in Nursing and in good standing with the Board.
• Must have at least two years’ recent experience in the direct care of the elderly or disabled adults
Physical Demands:
• Ability to be physically mobile (stand, walk, reach, climb, bend) and the ability to sit for prolonged periods throughout the day/work hours
• Manual dexterity sufficient to operate keyboard, copier, telephone, and the ability to write and/or type
• Must be able to occasionally lift and/or move up to 25 pounds
• Occasional driving to client appointments, offsite meetings, conferences, etc.
Other Requirements:
• Valid driver’s license
• Recent physical exam and TB screen within 12 months
