Position Summary:
Service Navigators carries out all the duties associated with direct case management on behalf of program clients under contract with DDS. In addition, this person is responsible for identifying and accessing resources to support and assist the program participants and their families in his/her caseload, and for maintaining direct communication with DDS direct and supervisory staff, as well as the Assistant Director of Family Support, and Director of Elder, Disability and Community Services.
Duties and Responsibilities:
- Intake & Assessment and Service Plan: In collaboration with DDS Service Coordinators and collaterals, upon being assigned a case, the Family Support Worker is responsible for conducting an assessment (Assets Inventory) on the client, and establishing a service plan with goals and objectives.
- Program Management: Responsible for maintaining program quality. This includes, but is not limited to participating in staff orientation and trainings, client performance and monitoring, and ensuring adherence to service plan.
The FSW is also responsible for maintaining accurate documentation of interactions with clients and family members, case collateral, and key events.
- Direct Service: Responsible for providing direct services to clients and their families. This includes, but is not limited to, skill building, service connection and navigation, information and referral, and transportation and interpreter services when appropriate.
- Special Projects: Expected to participate in agency-sponsored trainings.
Also, expected to participate in the planning and implementing of DDS calendar activities and special events, as well as industry networking and professional development activities, such as conferences and workshops that are provided.
- Outreach: Expected to conduct outreach activities among DDS Service Coordinators to ensure increased program utilization.
- Mandated Reporter Statute: An individual in this position is considered a “Mandated Reporter” when on, or off duty according to MA Statutes.

Education & Experience:
- Bachelor’s degree in Social Work, Human Services or related field preferred.

Physical Demands:
- Ability to be physically mobile (stand, walk, reach, climb, bend) and also the ability to sit for prolonged periods throughout the day/work hours
- Manual dexterity sufficient to operate keyboard, copier, telephone, and the ability to write and/or type * Must be able to occasionally lift and/or move up to 25 pounds
- Frequent driving to client appointments, offsite meetings, outreach events, conferences, marketing, etc.
