Position Name : Registered Nurse Hospice Admissions

Location : Troy MI 48083

Start Date : ASAP

Employment Type : Fulltime Travel Contract Temporary

Contract Length : 13 weeks contract Accepting Local and Travel Candidates

Pay Rate (Days) : Range $2200 to $2280 Weekly Gross pay (Nontaxable travel / housing and M&I stipend available for qualified / eligible travelers)

Unit : Home Health Hospice

Shifts available : Days

Shift timings : 10 hours; Days 10.00 am 8.30 pm

Schedule : 40 hours / week guaranteed 4 X 10hr shifts per week Mon Tue Thu Fri of each week Wednesday and Weekends off No call

Position Requirements :

  • Registered Nurse : 2 years Required
  • Recent / Current Hospice Nursing experience in a home health setting : 3 years Required
  • Must have experience with Admissions and Case Management Required
  • Home Care Home Base (HCHB) charting experience Required
  • Must have a valid Drivers License and Personal Auto Insurance
  • Must be Covid Vaccinated or provide a medical or religious exemption

Education License and Certification Requirements :

  • ASN or an equivalent Required
  • BSN or an equivalent Highly preferred
  • RN License for State of Michigan Required
  • BLS Certification from AHA or American Red Cross Required

    • Job Description :

  • Provide and coordinate care for assigned hospice patients in comfort of their homes.
  • Manage case load of up to 1418 patients and complete admissions
  • Develop and implement nursing care plans for assigned patient case load
  • Support patient comfort and ability to stay at home enabling them to spend quality time with their loved ones and doing the activities they enjoy.
  • Empower patients and families to reach healthcare goals by educating them on disease management.
  • Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations policies procedures and protocols as well as facility and corporate policies and procedures.
  • Ensure physicians orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status.
  • Document and complete charting for all patient visits within 24 hours.
  • Provide bereavement support to patients families as needed.
  • Territories : Float to all color teams
