Position Name : Physical Therapist Home Health

Location : Kettering, OH 45420

Start Date : ASAP

Employment Type : Full-time, Travel, Contract, Temporary

Contract Length : 13 weeks contract -Accepting Local and Travel Candidates

Pay Rate : Range $2320 to $2400 Weekly Gross pay (Non-taxable travel / housing and M&I stipend available for qualified / eligible travelers)

Mileage Reimbursement : $0.50 / mile for distance covered between patient home appointments

Unit : Home Health

Shift : 8 Hours; Days, 8.00a - 5.00p

Schedule : 40 hours / week guaranteed, five shifts per week, Mon-Fri

Position Requirements :

  • Current Physical Therapist experience : 1- 2 years - Required
  • Recent / Current Home Health PT experience of at least 1 year : Highly preferred

Education, License, and Certification Requirements :

  • Bachelor of Science in Physical Therapy or Similar - Required
  • Current State License as Physical Therapist or Compact if applicable - Required
  • BLS Certification from AHA or American Red Cross - Required

    • Job Description :

  • Provide skilled physical therapy services to patients in their homes as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to IDT team
  • Treats patient to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.
  • Counsel patient and family on the disease / injury and how to manage in all phases of physical therapy.
  • Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic / orthotic devices.
  • Oversee and supervise therapy services of patient provided by PTAs and when appropriate, nurse assistants.
  • Communicate with the PTAs and nurse aides on supervisory visit results.
  • Completes all clinical documentation following agency protocol and Medicare / Federal guidelines.
  • Must meet 30 productivity points per week
  • Territories to cover : Kettering, OH and surrounding areas
