Pay Rate: $33.75 - $45.00 / hour, depending on years of experience.
Centurion is proud to be the provider of comprehensive services to the Maryland Department of Public Safety and Correctional Services. We are currently seeking a Part-time Discharge Planner to join our Substance Use Disorder (SUD) team at North Branch Correctional Institution located in Cumberland, Maryland.
The SUD Program Discharge Planner is responsible for coordinating a patient’s release from the substance use program upon release or leave to another facility. This person is responsible for creating and maintaining relationships with referral sources and community agencies providing services to persons with substance abuse and co-occurring mental health disorders, as well as, identifying the need to return patients to referral source at the time of discharge.
Qualifications
- Bachelor’s Degree in Psychology, Social Work, Counseling, or related field from an accredited educational institution; Master’s Degree preferred
- Minimum of two (2) years of experience working in release planning preferred
- Clear written and verbal communication skills
- Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR)
- Corrections, forensic or criminal justice experience preferred
- Ability to obtain a security clearance, to include drug screen and criminal background check
