Position Summary

Full Job Description / Notes:

Protects, promotes and restores heath and wellness by practicing at the highest level of the Registered Nurse licensure; collaborating with providers and muLidisciplinary care team members to provide care coordination across the continuum. Assesses and supports physical symptoms, provides psychological support to patients, friends, and families, mentors assigned team members, promotes best practices, and maintains professional disposition at all times.

Required Qualifications

Education: High School Diploma/ GED

Experience: Current Registered Nurse licensure by the State Board of Nursing in appropriate state (Illinois or Michigan)

Licensure/ Certification: Current American Heart Association HeaLhCare Provider BLS is required prior to start date of employment.

Other Skills/ Knowledge: N/A PREFERRED QUALIFICATIONS:

Education: BSN

Experience: Experience as an RN in a rehab care setting Licensure/ Certification: N/A

Other Skills/ Knowledge: Electronic Medical Record and Microsoft application basic proficiency PRIMARY JOB RESPONSIBILITIES AND ACCOUNTABILITY:

Works at the highest level of licensure by providing qualiy competent care, following best practice standards, working to achieve care goals and qualiy outcomes, and following the standards of care set by the state board of nursing.

Provides effective leadership in the delivery of qualiy nursing care by assessing, planning, organizing, and directing the nursing functions and collaborating with the patient to implement their plan of care.

Maintains confidential status of medical records and maintains documentation standards with accurate, timely, and complete recording and reporting.

Acts autonomously as a vital component of the care team while delegating appropriate responsibiliy to the skill level of the employee and identifies patient care needs to accomplish a high standard of care in collaboration with the interdisciplinary team functioning as an advocate for those we serve.

Creates compassionate, service-oriented environment by following the nursing process in providing emotional, psychological and spiritual support to patients and caregivers.

Provides a learning atmosphere by sharing knowledge with coworkers/orientation of new staff and by educating patients and their families to meet their heath needs.

Participates in our qualiy and process improvements to attain clinical excellence. Promotes financial stewardship by assisting with cost reduction without sacrificing qualiy.

Maintains a safe and clean working environment and complies with policies, procedures and protocols put in place to protect patients and Partners.

Enhances professional growth and development by participating in and utilizing current evidence based information and keeps abreast of state and national accreditation, regulatory and safer standards.

Advocates for patients and their caregivers to positively influence value driven and effective nursing care within the heaLhcare continuum.

Continually traversing corridors. Must occasionally remain in a stationary position, sitting or standing, for up to 4 hours. Operates of ce and medical equipment constantly. Frequently lifts/moves/carries objects/persons up to 35 pounds. Occasionally must position self to lower heights under 4, stooping or squatting. Near acuiy vision needed. Constant coordination of hands, fingers, feet and eyes required. Completes basic math quickly and accurately. Understands complex problems and explores aLernative solutions. Organizes thoughts and ideas into understandable terminology. Constantly working with others, must converse and be understood verbally and in written form. Occasionally must work to understand and negotiate and/or integrate different viewpoints. Continual decision making based on data and observation required. The environment includes frequent emotional stress and occasional violent or assauLive customers. Hazards include exposure to blood borne pathogens, contagious diseases and chemical exposure.
