Founded in 2014, Certintell is a Care Management company that offers programs to Medicare, Medicaid, and Dual-eligible patients in partnership with medical clinics, pharmacies, and insurance providers. Certintell acts as a virtual care clinic by providing programs that improve access to care, reduce the cost burden of chronically ill patients, increase revenues to the Health Center, and can improve overall Value-based Payments (VBP) and other relevant outcome-driven ratings (e.g. HEDIS).

Provides rehabilitation services at the Pediatric and Adolescent Transformative Health (PATH) Clinic. PATH is a 12-member collaborative and will provide an innovative structure for a Behavioral Health Home (BHH) serving children, adolescents, and families on the west and southwest sides of Chicago. Evaluates and treats patients using the principles and practices of physical therapy in accordance with the patient's age, developmental level and diagnosis.
*This position requires occasional travel to clinics*
Essential Functions (list in order of importance including measures)
- Evaluates patients, establishes goals and develops individualized treatment plans.
- Provides direct patient treatment based on current physical therapy practice and revises treatment plan based on the patient's response.
- Provides education to patient and family regarding patient abilities and need for assistance and continued therapeutic intervention.
- Consults with physicians on patient's evaluations, progress, and complications with treatment.
- Maintains medical and departmental records according to department policies and procedures.
- Participates in patient care conferences, department meetings and department and hospital in-services.
- Communicates with other professional staff and patient's family/support system to provide continuity of care.

- Participate in community outreach programs and wellness initiatives.

- Doctorate in physical therapy
- Illinois PT license
- CPR Certification
- Bilingual English/Spanish a plus
- Ability to communicate with staff at all organization levels

Pay: $50.00 - $60.00 per hour
Expected hours: 20 – 30 per week
