Summary
We are seeking a Licensed Clinical Social Worker to join our team at Candlewood Healthcare & Rehabilitation. In this role, you will provide essential support and counseling services to residents, helping them navigate challenges and improve their overall well-being. Your expertise will be vital in creating a nurturing environment that aligns with our mission of family-focused care.

Working at Candlewood Healthcare & Rehabilitation allows you to work with this population as well as other short term and long-term populations to provide social services needed to those looking to return to the community.
This position will work Monday through Friday as part of Candlewood’s management team. Start and end times are flexible. As part of the management team you will be part of a weekend manager on duty rotation.
Starting negotiable rate for the position is $65,000.00 annually

Responsibilities
- Conduct assessments and develop individualized treatment plans for residents.
- Provide crisis intervention and support for individuals facing emotional or behavioral challenges.
- Collaborate with multidisciplinary teams to ensure comprehensive care for residents.
- Supervise support staff and provide mentoring to enhance team effectiveness.
- Maintain accurate documentation of patient progress and treatment outcomes.
