Position requirements:
- We prefer a minimum of 4 years of experience in assisted living or skilled care environment in a supervisory capacity to include experience with scheduling, training, and regulatory compliance.
- Develop an individualized Assisted Living service plan for each resident by conducting a Health Care Evaluation and soliciting input from the primary care physician, resident, and/or Responsible Party upon move-in to the community.
- Reevaluate the individualized Assisted Living service plan for each resident no less than bi-annually and at the time of a significant change in health condition to ensure appropriate care levels are being captured always.
- Communicate with the Executive Director on a weekly basis regarding the appropriateness of residents participating in Assisted Living services; communicate any significant changes in health condition immediately.
- Ensure Wellness Supervisors (LPNs) and Med Techs are completing medication administration and documentation as outlined in the state regulations.
- Hire and schedule the Wellness Supervisors (LPNs) and Wellness Associates (CNAs) within budgetary guidelines; provide supervision, training, performance review, and counseling as needed for these associates.
- Supervise Wellness Associates, Med Techs, and Wellness Supervisors.
- Maintain compliance to all local, state and federal regulatory guidelines; participate in local and state inspections as appropriate; implement any corrective actions needed because of local/state inspections.
- To supervise the delivery of assistance with activities of daily living as outlined in Assisted Living service plans, ensuring the delivery of assistance is performed in a dignified, compassionate manner that also encourages as much independence from the resident as possible.
- Licensed Practical Nurse license as specified by State agency.
- 2 years of experience in assisted living or skilled care in a supervisory capacity to include experience with scheduling, training, budget management, and regulation compliance.
- Excellent customer service skills; strong verbal and written communication skills; ability to handle multiple tasks and work effectively with team members.
