Position: Case Manager – Intake Center
Location: Intake Facility (7226 Figueroa St, Los Angeles, CA 90003)
Reports to: Intake Facility Manager
Summary
The Case Manager at Ceryx Management’s Intake Center is responsible for processing incoming participants, assessing their needs, and coordinating services to ensure they receive the appropriate care and support. This role is critical in connecting clients with short-term housing and essential recuperative care while supporting their transition to stability.
Key Responsibilities
1 Participant Intake and Assessment
- Conduct thorough intake interviews with incoming participants, gathering medical, social, and housing histories.
- Assess eligibility for recuperative care services in alignment with CalAIM and Soul Housing criteria.
- Develop initial care plans tailored to participants' medical and social needs.
2 Coordination of Services
- Collaborate with healthcare providers, case managers, and facility staff to ensure participants are placed in the most appropriate facility.
- Facilitate the handoff of participants from the intake process to designated care teams at assigned facilities.
- Identify additional support services (e.g., mental health resources, transportation, or specialized care) and make appropriate referrals.
3 Documentation and Compliance
- Maintain accurate and detailed records of participant intake, assessments, and referrals.
- Ensure compliance with HIPAA, CalAIM guidelines, and Soul Housing policies.
- Prepare and submit regular reports on intake metrics and outcomes to management.
4 Participant Advocacy
- Act as an advocate for participants, ensuring their voices are heard during the care planning process.
- Address any immediate concerns or barriers to entry, such as lack of documentation or mobility challenges.
5 Team Collaboration
- Work closely with intake staff, including medical professionals, administrative personnel, and operations teams.
- Participate in team meetings, training sessions, and case reviews to ensure consistent care quality.
6 Crisis Intervention
- Respond effectively to participants in crisis, providing de-escalation and connecting them with emergency resources as needed.
- Ensure participants are welcomed with dignity and compassion, regardless of their circumstances.
Qualifications
- Education:
- Bachelor’s degree in Social Work, Psychology, Public Health, or a related field.
- Master’s degree preferred.
- Experience:
- Minimum of 2 years of case management experience, preferably in a housing, healthcare, or social services setting.
- Experience working with homeless populations, individuals in crisis, or in an intake environment is a plus.
- Skills/Competencies:
- Strong interpersonal and communication skills, with the ability to work effectively in a fast-paced environment.
- Knowledge of local social service agencies and resources.
- Proficient in case management software and documentation processes.
- Ability to balance multiple priorities and work collaboratively with a multidisciplinary team.
Work Environment
- Schedule: Full-time role, Monday through Friday, with occasional evenings or weekends as needed.
- Physical Requirements: Ability to sit, stand, and move within the facility for extended periods.
- Environment: Office setting with direct interaction with participants in need of immediate care and housing.

Pay: $26.00 - $30.00 per hour
