We are seeking a skilled and compassionate Social Worker to join our healthcare team in a full-time capacity. As a Social Rehabilitation Specialist, you will play a vital role in planning and delivering social rehabilitative programs, collaborating with the treatment team to develop comprehensive care and treatment plans for the youth population.
Key Responsibilities
- Client-Centered Approach: Build and maintain strong rapport with patients, fostering a trusting and supportive environment through individual and group therapy sessions.
- Clinical Expertise: Conduct comprehensive biopsychosocial evaluations, provide clinical services, including assessments, screening, brief therapy, and community referrals.
- Treatment Planning: Collaborate with the treatment team to develop and implement individualized treatment plans that adhere to current policies and procedures.
- Counseling: Conduct one-on-one counseling with clients to help them prepare for their release, address personal issues, and develop crucial life skills.
- Documentation: Thoroughly document all services provided in medical records, ensuring accurate and organized records of patients' progress.
Requirements
- Masters degree required.
- Must possess knowledge of the judicial, correctional, and mental health systems, clinical theory and practice with emphasis on crisis intervention, and community resources and how to access them.
