POSITION SUMMARY
Responsible for the planning and delivery of social and addiction rehabilitative programs in the community post discharge. Coordinates and participates with the treatment team in the development of a comprehensive discharge plan of care and treatment. Participates and coordinates discharge and release planning for inmate patients in the facilitys medication assisted treatment (MAT) program.
POSITION RESPONSIBILITIES

Program involves activities such as:
- Manual task skills
- Practice in functional, prevocational, vocational, cell sanitation, self-care skills and activities of daily living
- Participation in sensor-motor, educational, recreational and social activities designed to help inmate patients in the MAT program.
- Coordinates the development and implementation of treatment plans for assigned inmates as consistent with current MAT policies and procedures
- Counsels inmates by interviewing them to ascertain their needs prior and after release.
- Selects constructive activities suited to the individuals physical capacity, intelligence level and interest to upgrade the individual to maximum independence
- Counsels on a personal level and advises the individual inmates to assist them in preparing for their release.
- Appropriately provides documentation in the Medical Records any changes in the inmates functioning, the occurrence of critical incidents, group counseling & individual counseling, services provided.
- Consults with Department of Health, Grant Program Directors, physicians office, halfway houses and other agencies to coordinate discharge planning efforts.
- Participates in and contributes to the planning of training activities
- Maintains regular contact with the supervisor
- Assumes personal responsibility for ongoing professional growth and development
- Assist in development, support and progress of service plan coordination for MAT participants.
- Develop rehabilitative and discharge methods unique to population and program goals.
- Maintain accurate progress notes and data identifying needs, strengths and gaps in services.
- Assist MAT participants navigate accessibility to obtain Medicaid/insurances.
- Accomplish timely and appropriate discharge plans through assessment of the patients post-release needs and the provision of services necessary to meet those needs.
- Collaborate and coordinate with all applicable parties (e.g., social work, medical, security) for release planning.
- Update and maintain accurate information and records related to the patients discharge planning, including active files of available community resources to facilitate the release planning
- Develop database and files of available community resources to facilitate the discharge planning process.
- Deliver care in a nonjudgmental/nondiscriminatory manner to protect the autonomy, dignity and rights of the patient
- Understand and comply with confidentiality, safety and security rules and regulations of the institution.
- Complies with the employment standards of CFG Health Systems and the applicable standards of the facility
- Complies with the time and attendance policies of CFG Health Systems. This includes he use of the Swipe System and timely submission of time sheets
- Adheres to his/her work schedule as coordinated by Health Services Administrator.
- All other duties as assigned
POSITION REQUIREMENTS
- Bachelors Degree in Counseling, Social Work or related field, at least 2 years of mental health/residential experience preferred.
- Must conform to the security clearance standards of the institution where employed.
