The ideal candidate will assist the Case Manager Director in organizing, planning, and directing the work of building team members to support the progression of each patient case. This position requires strong organizational skills, the ability to work collaboratively with other healthcare professionals, and a dedication to ensuring exceptional care for our patients.
Job Summary:
The primary purpose of the Case Manager Assistant position is to assist the Case Manager Director with planning, organizing, and directing building team members to take the necessary steps to support each patient’s case progression. The Case Manager will work closely with Social Workers, Clinical Team Members, and Therapists, and collaborate with each patient's assigned insurance case manager.
Key Responsibilities:
Verify that daily charting is appropriate and meets coverage guidelines.
Obtain appropriate therapy, coinsurance, room and board, medication, and treatment authorizations.
Communicate effectively with building team members about when ABNs and NOMNCs must be issued.
Manage clinical data collection for timely insurance updates, including completion of appropriate insurance forms.
Ensure services logs are submitted to insurance companies as patient cases come to a close.
Complete insurance appeals as needed.
Notify Insurance Case Managers of any resident change of condition, admission, and/or discharge.
Review the content of each patient’s case file during weekly Medicare meetings.
Upload Insurance Authorizations to PCC.
Qualifications:
Strong organizational and communication skills.
Knowledgeable in medical terminology.
Minimum of 2 years of experience in long-term care preferred.
Professional Requirements:
Adheres to dress code, appearance is neat and clean.
Completes annual education requirements.
Maintains regulatory requirements, including federal, state, and local regulations and accrediting organization standards.
Ensures resident confidentiality at all times.
Reports to work on time and as scheduled and is willing to travel as needed.
Wears identification while on duty.
Attends annual reviews and performs departmental in-services.
Works at maintaining a good rapport and cooperative working relationships with physicians, departments, and staff.
Represents the organization in a positive and professional manner.
