Admissions Director – Weekend Position (Saturday & Sunday, 8 : 00 AM to 5 : 00 PM)
Location : Muskego, WI
Join our team at Champion Care as an Admissions Director! This weekend position is based at our centralized admissions hub office in Muskego, WI, where you will play a vital role in assisting with the admissions process for skilled nursing facilities across WI, TN, and NJ.
As the Admissions Director, you will work remotely outside of the nursing facility, coordinating the admissions process for new patients. From reviewing referrals to working closely with hospital discharge planners and families, you’ll ensure a smooth transition and welcoming arrival to our facilities.
Key Responsibilities :
- Direct the admissions process and ensure appropriate patient placement, including coordinating admission documentation.
- Virtually collaborate with facility sales and marketing teams to enhance admissions efforts.
- Ensure room readiness and optimal bed availability for incoming residents.
- Assess potential residents for clinical and financial appropriateness to ensure seamless transitions.
