GENERAL DESCRIPTION
Champion Services provides support in the Home and Community (HCS) setting for adult individuals with intellectual and developmental disabilities (IDD).The Support Specialist is responsible for planning, coordinating and monitoring identified services for the individuals on their caseload who are in the Home and Community-Based Services (HCS) Medicaid Waiver Program. The Support Specialist will report to the Client Services Manager and provide input to the direct supervisor of Direct Care Staff and Foster Care/Companion Care Providers.
Job Duties & Responsibilities
Develop and monitor the Implementation Plan and the Individual Plan of Care to achieve the individual’s personal outcomes as identified in the Person Directed Plan, reflects the results of the assessments of the individual’s strengths and his or her family strengths, and the individual’s needs.
Ensure all required documentation is in compliance with agency, state and federal guidelines as well as the HCS Principles and Billing Guidelines in order to maximize state reimbursements and outcomes of the annual waiver certification and billing audits.
Skills & Specifications
- Knowledge of HCS Principles, PDP & IP process, HCS Billing Guidelines and related software
- Organized, detail oriented, and willing to help others excel
- Effective communication skills (written and oral communication)
- Proficient in the use of Microsoft Office Software.
- Able to appropriately receive and give corrective feedback

- Ability to provide 24 hour on call accessibility.

Education, Qualifications & Certifications
a) Bachelor’s degree in a Human Services, Psychology or related field
b) 3 years of experience providing case management for individuals with Intellectual and Developmental Disabilities (IDD)
Job Type: Full-time
Pay: $16.75 - $19.60 per hour
