Case Manager - Bilingual English/Spanish P3, Full-time

Position Summary:   This is a full-time direct services position in our Positive Parenting Partnerships (P3) program and is responsible for delivering family support services. P3 case management services provides family support services utilizing a broad range of community services and natural support systems. This position reports to the Case Manager Supervisor. 

Wage rate: $20.51 per hour with a 37.5 hour work week.  Approximately $40,000 annually. 

This is a full-time regular position. Position includes benefits such as paid time off, paid holidays, 100% employer paid health insurance, 401k and much more. 

ESSENTIAL DUTIES:

1. Research, design and deliver case management services within the P3’s framework. 
   1. Coordinate and collect required assessment and measures.
   2. Complete an initial client contact and/or assessment, within the specified program timeframes and guidelines, to determine client needs and develop a plan that supports those needs.
   3. Gathers appropriate referral and assessment information for P3 Team.
   4. Monitors ongoing progress and needs within the family and all supportive services.  Serves as liaison to connect the family and all involved agencies and systems to include financial health supports.
   5. Links families to services and natural support systems as stated in the Family Support Plan.
   6. Coordinates existing and added services.
2. Cultivate and manage ongoing collaborative relationships with the early childhood and family serving community. 
3. Complete accurate and timely data collection to complete reporting requirements.
4. Attend staff meetings, on-going in-service training and supervision as required.
5. Participate on appropriate agency, project and community planning teams.
6. Performs other related duties as assigned.

REQUIREMENTS:

• Minimum Bachelor’s Degree in social work, human services or related field.
• Minimum of 3 years relevant experience working with families.
• Case Management Credential preferred
• Ability to read, write, and speak English and Spanish is required.
• Manage a caseload; organize and prioritize work assignments; make decisions and independent judgments; determine the appropriate course of action in emergency or stressful situations; and recognize reportable events.
• Computer skills with proficiency in Microsoft Office (Excel, Word, Outlook) and internet.
• Ability to work in the office, clients’ homes and in the field.
• Excellent verbal and written communication skills
• Professional, self-directed, follow-through on projects and prompt responsiveness to internal and external stakeholders.
• Good interpersonal skills, team-oriented, customer service focused enjoys working with others. 
• Strong verbal communication skills and demonstrated ability to write clearly and persuasively
• Detail oriented, with excellent organization skills required. 
• Light physical demands: Regularly required to sit; use hands to finger, handle or feel; reach with hands and arms and talk and hear. Ability to walk, stand, climb stairs, kneel, bend, reach, and manipulate objects; lift and move materials up to 10 pounds on a regular basis; infrequently require to lift or move materials weighing up to 20 pounds.
• Some travel in the communities served; must have a reliable car, active auto insurance, and valid Florida driver’s license. 
• Must be sensitive to the socioeconomically and culturally diverse population Champions for Children serves.
• Must be able to successfully complete a Florida Level 2 criminal background check, motor vehicle records check and drug screening.
