SUMMARY: The Family Champion (Case Manager) is a full-time direct services position responsible for delivering family support services and case management services to families in pediatric care and using a broad range of community services and natural support systems. As the case manager, this individual will identify needed community resources and supports for DULCE families and act as a back-up for families with more complex needs. The Family Champion (Case Manager) will then coordinate scheduling and support for follow up consultations, as well as partake in data collection, administrative support, and follow up with referrals and connections for the DULCE families

Wage rate: Range: $18.97 to $22.05 per hour; 37.5 hours per week schedule. Approximate annual: $37,000 to $43,000

Job class: Full-Time, Non-Exempt

This position will be located at the service location: Tampa Family Health Center, 4620 N. 22nd St,, Tampa, FL 33610

This position reports to the Lead Family Specialist. Specific responsibilities include, but are not limited to:

ESSENTIAL DUTIES:

  1. Research and deliver case management services within the DULCE’s program framework when needed.
  1. Complete an initial client process, coordinate and collect required screening and/or assessments, within the specified program timeframes and guidelines.
  2. Link families to services and natural support systems.
  3. Monitor ongoing progress and needs within the families. Serve as liaison to connect the caregivers to involved agencies that can provide support.
  4. Coordinate existing and find new services for participating families.
  1. Cultivate and manage ongoing collaborative relationships with the early childhood, child welfare, behavioral health, legal, housing, and family serving community.
  2. Implement the DULCE services and work with families to develop a Family Support Plan and Program Transition Checklist.
  3. Provide caregiver education, tools, and resources to DULCE families.
  4. Timely data entry into participant records and data collection systems.
  5. Prepare for and participate in case reviews with the Interdisciplinary Team.
  6. Adheres strictly to policies which ensure participant confidentiality.
  7. Participates in and supports the agency’s performance and quality improvement processes.
  8. Participates in related training and workshops.
  9. Performs other related duties as assigned or requested.

REQUIREMENTS:

  1. Minimum bachelor’s degree in social work, human services, or related field. AA degree with direct relevant experience working with newborns and infants may be considered.
  2. Preference will be given to individuals with 5 years prior experience in supporting families with newborns by connecting them to support services.
  3. Case Management Credential preferred but not required.
  4. Manage a caseload; organize and prioritize work assignments; make decisions and independent judgments; determine the appropriate course of action in emergency or stressful situations; and recognize reportable events.
  5. Bi-lingual (English/Spanish) preferred, but not required.
  6. Computer skills with proficiency in Microsoft Office (Excel, Word, Outlook), Microsoft 365 web-based applications and using internet search engines and other online research tools.
  7. Ability to work in the office and in the field.
  8. Available to work evenings and Saturdays and schedule visits according to participants needs.
  9. Professional, self-directed, high-energy can-do attitude, follow-through on projects and prompt responsiveness to internal and external stakeholders.
  10. Good interpersonal skills, team-oriented, positive customer service focused enjoys working with others.
  11. Light physical demands: Regularly required to sit; use hands to finger, handle or feel; reach with hands and arms and talk and hear. Ability to walk, stand, climb stairs, kneel, bend, reach, and manipulate objects; lift and move materials up to 10 pounds on a regular basis; infrequently require to lift or move materials weighing up to 20 pounds.
  12. Able to travel in the communities served; must have a reliable car, active auto insurance, and valid Florida driver’s license.
  13. Must be sensitive to the cultural and socioeconomic characteristics of the population that Champions for Children serves.
  14. Must be able to successfully complete a Florida level 2 criminal background clearance, motor vehicle records check and drug screening.
