Position Summary: The Family Champion (Case Manager) is a part-time (32 hours per week with some flexibility for evenings and Saturdays) responsible for connecting with fathers, creating, and managing a family support plan where concrete supports are provided by utilizing a broad range of community services and natural support systems. This position reports to the Lead Family Champion.
Fall internship approximately 14.5 weeks, 32 hours per week. wage rate: $15.00 per hour.
ESSENTIAL DUTIES:
- Deliver case management services within the FRANC framework under supervision.
- Coordinate and collect required assessment and measures.
- Complete an initial client contact and/or assessment, within the specified program timeframes and guidelines, to determine father’s needs and develop a plan that supports those needs.
- Monitors ongoing progress and needs within the father and his family and all supportive services. Serves as liaison to connect the father and all involved agencies and systems to include financial health supports.
- Links fathers to services and natural support systems.
- Coordinates existing and added services.
- Cultivate and manage ongoing collaborative relationships with the early childhood, child welfare, behavioral health, legal, housing, and family serving community.
- Timely data entry into participant records and data collection systems.
- Prepare for and participate in case reviews with the Interdisciplinary Team.
- Participate in quality improvement activities.
- Participate in outreach and community events.
- Participate in related training and workshops.
- Perform other related duties as assigned or requested.
- Strictly adhere to policies which ensure participant confidentiality.
REQUIREMENTS:
- Student intern in social work, human services, or related fields.
- Bi-Lingual (English/Spanish) preferred.
- Learn to manage a caseload; organize and prioritize work assignments.
- Computer skills with proficiency in Microsoft Office (Excel, Word, Outlook) and internet.
- Ability to work in the office, virtually and in the field.
- Ability to work some evenings and weekends.
- Professional, self-directed, follow-through on projects and prompt responsiveness to internal and external stakeholders.
- Good interpersonal skills, team-oriented, customer service focused enjoys working with others.
- Detail oriented, with excellent organization skills required.
- Light physical demands: Regularly required to sit; use hands to finger, handle or feel; reach with hands and arms and talk and hear. Ability to walk, stand, climb stairs, kneel, bend, reach, and manipulate objects; lift and move materials up to 10 pounds on a regular basis; infrequently require to lift or move materials weighing up to 20 pounds.
