Description
Under the direct supervision of the Program Manager, the Case Manager will manage and oversee a variety of intensive case management functions within the Parenting and Permanent Supportive Housing Program, which consists of case management duties. The Case Manager will deliver services in both a group office setting to provide the participants with skills, knowledge, resources, and support via psychoeducation, case management, and the application of nurturing parenting to help equip the participants of the program to make informed and prosocial decisions. Ensure compliance with state and federal laws and regulations, in addition to the policies and procedures set forth by Champions and maintain appropriate documentation within the electronic client records, which includes the use of an Electronic Health Record (EHR) system. Furthermore, work collaboratively with all divisions within Champions and provide integrative treatment services, individual and Psychoeducational groups and classes as deemed appropriate. The Case Manager will ensure proper and ethical documentation for services and comprehensive service delivery to clientele.
- Perform periodic inspections of the Housing and Urban Development (HUD) homes and develop corrective action plans to address problems.
- Makes home visits in connection with case work assignments.
- Complies with all program policies and procedures, including State / Federal requirements.
- Maintains a caseload of clientele and provides active and comprehensive case management services. Ensuring all area needs are attended to (mental, substance abuse, social, physical, and family).
- Ensures appropriate and comprehensive documentation within an EHR system.
- Monitoring the health and welfare of the consumer and the quality of services provided to the consumer through home visits and telephone calls.
- Participates in staff meetings, addressing client needs, program issues, and assisting in the integration of services for clientele.
- Facilitate Psychoeducational groups and classes, as deemed appropriate by management.
- Provides integration of all service divisions of the program, specifically bridging disciplines like mental health and probation.
- Develops and maintains cooperative work relationships with community agencies and partners, including client connections to outside agencies (i.e., court hearings, open Child Protective Services (CPS) cases and requirements, etc.)
- Determines appropriate services available through the Parenting program and / or referral to other community agencies.
- Complete intakes and eligibilities for potential clientele and verify completeness and accuracy of documents.
- Serve as the main point of contact for all clients and relay client needs to the appropriate channels.
- Participate in applicable meetings, trainings, conferences, etc. as required both internally and externally.
- Remains culturally sensitive and respectful in all client and staff interactions.
- Adheres to all client confidentiality requirements and standards, including updates of the shared Release of Information (ROI) to ensure that information is current and accurate.
- Comfortable performing various projects in conjunction with day-to-day activities.
- Ability to have a good relationship with diverse personalities, understanding, mature, and flexible.
- Possess strong judgment, problem-solving and reasoning abilities.
- Complete intakes and eligibilities for potential clientele and verify completeness and accuracy of documents.
- Participate in applicable meetings, trainings, conferences, etc. as required both internally and externally.
- Adheres to all client confidentiality requirements and standards, including updates of the shared Release of Information (ROI) to ensure that information is current and accurate.
- Conducts urinalysis and breath testing of clients as needed.
- Conducts community outreach on Champions services when needed.
- Comply with all Champions policies and procedures, including workplace safety, reporting work-related injuries, Infection Control and preventing potential safety risks for staff, clients, and others.
- Performs other duties as assigned.
