Outpatient LMHC/LMSW

Champlain Valley Family Center is looking for a licensed therapist for our expanding behavioral healthcare outpatient clinic. This clinic works with individuals with mental health and substance use disorders. The individual is responsible for individual and group counseling, assessments, treatment planning, collaborating with outside providers, and working together with a treatment team to meet the needs of any clients. CVFC has competitive pay and great benefits. Applicants must have an active LCSW, LMSW, or LMHC. Pay is based off experience.

Position Overview:
We are seeking a Licensed Mental Health Counselor (LMHC) or Licensed Master Social Worker (LMSW) to join our dynamic team. The ideal candidate will be passionate about integrated healthcare and committed to providing exceptional care to our patients. As a practitioner at our clinic, you will collaborate with our multidisciplinary team to assess, diagnose, and treat individuals with mental health concerns.

Responsibilities:

• Conduct comprehensive mental health assessments
• Develop and implement individualized treatment plans
• Provide evidence-based psychotherapy to individuals, groups, and families
• Collaborate with other healthcare providers to ensure coordinated care
• Maintain accurate and timely clinical documentation
• CVFC strives to provide quality care, limiting caseloads to ensure thorough and personalized care. (Max 40)

Qualifications:

• Master's degree in Counseling or related field
• Current LMHC/LMSW license in NYS
• Experience working in an integrated healthcare setting preferred
• Strong communication and interpersonal skills
• Ability to work effectively in a team environment
