Serves as a leader of the Community Health Worker (CHW) team. Conducts outreach to implement programs in the community that promote, maintain, and improve individual and community health. May provide information on available resources, provide social support and informal counseling. Advocate for individuals and community health needs, and provide services such as blood pressure screening. May collect data to help identify community health needs

Job Summary:

Responsibilities:
- Prepares external and internal communication and related material for program/department asrequested from verbal direction or from knowledge of systems policy and/or procedure.
- May coordinate and lead evidence based community health programs.
- Partner with other Community Health Workers and community health organizations, agencies in the community.
- Serve as lead program contact when supervisor is unavailable. Assist in completing and submitting program deliverables.
- Educates designated clinics and departments about CHW program, meets all qualifications outlined in the Community Health Worker (CHW) job description.
- Serve as a liaison, or as a bridge, between participants, healthcare providers, hospital staff, and community/state resources. Attend community meetings and health fairs to understand community needs and fosterrelationships with partners and community members. Connects with resources available to assist the poor and vulnerable to provide patient care. Provide culturally sensitive health education and information sessions.
