Reports To: Senior Clinical Program Manager
The Connecticut State Mental Health Waiver, Community Support Program (CSP); consists of mental health and substance abuse rehabilitation services and necessary support to assist the individual in achieving and maintaining the highest degree of independent functioning in a non-office setting. The service utilizes a team approach to provide intensive, rehabilitative community support, crisis intervention, group and individual psychoeducation, goal setting, and skill-building activities for daily living. Community-based treatment enables the team to become familiar with the participant's surroundings, strengths, and challenges. The team services and interventions are highly individualized and tailored to the needs and preferences of the individual.
Our directive, as a direct-care staff member of the CSP department, is to provide service of at least 1-3hours weekly to the participant in their home and/or other community settings.
These services include:
Assess, develop, adjust, and re-evaluate rehabilitation plan
Respond to crisis (face-to-face or telephonically)
Provide psychiatric or substance abuse disorder psychoeducation
Clarify goals, build skills and provide support related to; self-care, finances, shopping, leisure time, communication, transportation, nutrition, housekeeping, employment, education, community involvement, housing needs, personal safety, parenting, first aid, and problem-solving.
Teaching of recovery skills to prevent relapses such as symptom recognition, coping with symptoms, emotional management, relaxation skills, self-administration and appropriate use of medications, and preparation of illness-related advance directives.
Development of skills for the purpose of accessing natural supports, self-help, and other advocacy resources; and health and wellness education.
Maintain and upkeep documentation of each participant receiving work or education-related services

Qualifications:
Candidate shall hold either a bachelor?s degree in a behavioral health-related specialty (may include special education or rehabilitation) OR hold a High School diploma or equivalent and have two years? experience in the provision of mental health services (may include special education and/or services to persons with developmental disabilities) OR be a Certified Peer Specialist and possess:
Willingness to work nights and/or weekends as needed
Reliable transportation, valid driver's license, and proof of insurance
Strong organizational skills, ability to multitask and manage time appropriately
Ability to work independently or with a team
Ability to market services; network, negotiate with and develop community resources
Knowledge of Microsoft Office
Effective verbal and written communication skills
$17-$18/hr (depending on experience)
Part-time to start
