What are the responsibilities and job description for the Community Case Manager position at Chapman Partnership?
SUMMARY :
Community Case Management Services will be guided by ethical principles respecting autonomy, dignity, privacy and the rights of the individual. Every Chapman Partnership Case Manager provides case management services to residents and families with children to meet their social and emotional needs as they impinge on their medical, social, psychological, economic, and spiritual condition, and the safe transition from one environment to another.
DUTIES AND RESPONSIBILITIES :
- Assist Hotel / Motel families for appropriate services and formalize individual service and goal plans.
- To complete assessments within 72 hrs. of admission into Hotel / Motel (except weekends).
- Participate in case conferences regarding resident's status outplacement / discharge plan.
- To provide services in the community to restore family to optimal social environment and utilize community agencies and resources to help resident stabilize and regain independence.
- To maintain current case notes and goal plans for each family assigned and make regular entries regarding client's activities.
- To provide employment / housing assistance and follow-up services or to facilitate transfer to shelter.
- To assist family with housing plans and refer to follow up case manager for further planning and referrals.
- To prepare and keep current case records for each family; prepare and complete statistical and variety of reports for Chapman; participate in customer satisfaction surveys; prepare and analyze productivity and performance reports.
- Performs other assignments or tasks as deemed necessary.
