Description

POSITION SUMMARY: The Behavioral Health Navigator will help to identify patients with behavioral health needs and provide necessary assessments and linkage to outpatient and in-patient care needs. The Behavioral Health Navigator will be responsible for psycho-social education, pre-screening/ risk assessments as well as crisis support services within CDHC, partner community systems and as a part of a multi-disciplinary team approach. Other duties will include using evidenced based screening tools to deliver early intervention, treatment and linkage services up to inpatient hospitalization as indicated. This position is a part of the primary care treatment team and will help to triage and identify patient behavioral health issues. The Behavioral Health Navigator provides leadership in facilitating community-based support with an emphasis on supporting all clinical departments of CDHC to include Medical. Behavioral Health and Dental. Other duties will include using evidenced based screening tools to deliver early intervention, treatment and linkage services up to inpatient hospitalization as indicated.

 

POSITION-SPECIFIC COMPETENCIES/ESSENTIAL FUNCTIONS/DUTIES & RESPONSIBILITIES:

  • Assist Behavioral Health providers in helping patients and their families meet their individual needs
  • Completes clinical documentation according to internal CDHC policies and procedures and in accordance with regulatory bodies, including but not limited to, Nebraska Department of Health and Human Services, Joint Commission, CMS, and Region 6.
  • Work with primary care team to treat and manage patients with chronic emotional and/or mental health problems
  • Assist in the detection of “at risk” patients and the development of relapse plans to prevent further psychological or physical complications
  • Attend and participate in required internal and external meeting and activities
  • Work diligently and professionally with internal and external partners
  • Assist with the integrated model and Warm Hand Off’s at any of our service delivery sites
  • Assist in implementing CDHC programming initiatives
  • Serve as a liaison connecting the dots between the community and CDHC. Coordinate outreach, and education activities for relevant behavioral health issues
  • Track outcomes and provide reports as needed
  • May provide supportive services and psychoeducational interventions primarily in patient’s natural environment.
  • Other duties as assigned

Qualifications


POSITION REQUIREMENTS

  • Education: Bachelor’s Degree Preferred
  • Licensure: Provisional Licensed as a Mental Health Therapist in the State of Nebraska (PLMHP) preferred.
  • Experience: 4 plus years of experience working with youth, adults and or families preferred
  • Expertise: Working knowledge of evidence-based treatments for mental health conditions.  Ability to make quick and accurate clinical assessments of mental and behavioral conditions.  Ability to work through brief patient contacts, including via telephone and teleconferencing.  Ability to train and teach effective behavioral health interventions.  Ability to work effectively as a team, interfacing with patients, primary care providers, consulting psychiatrist and mental health specialists, as well as with administrative and support staff.  Ability to effectively represent CDHC’s interests in the community and maintain effective working relationships among co-workers, public, private, and professional groups.  Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Must be comfortable with use of computer operating systems (i.e. Microsoft Word, Excel, PowerPoint) and Electronic Health Record System.
  • Language: English, bilingual is a plus
  • Work Hours: Typically, Monday – Friday between the hours of 8:00am – 6:00pm, will also include some evenings and weekends for outreach events.
  • Travel: Locally, intrastate, and interstate, as necessary
  • Exposure: The exposure characteristics described here are representative of those an employee encounters while performing the essential functions of this position.  For example, while performing the essential functions of this job, the employee is rarely exposed to fumes or airborne particles and toxic or caustic chemicals.  Additionally, the employee may occasionally be exposed to blood borne and other hazardous chemicals.  Finally, the noise level in the work environment can occasionally be quite loud.  In all cases personal protective equipment will be provided to the employee in combination with adequate ventilation and other engineering controls to minimize the risk of exposure or other hazardous occurrence.
