INTRODUCTION: The Program Director of In-Home Supports provides program management and development of the In-Home Supports Program. The In-Home Supports program provides community-based, individualized person-centered services to individuals who live independently in the community and benefit from care coordination, skill development, community inclusion, and assistance with activities of daily life to maintain the highest level of autonomy and independence.

The Program Director ensures that all aspects of In-Home Supports are carried out in manner consistent with Charles River Centers policies and guidelines set forth by the Department of Developmental Services (DDS). This includes, but is not limited to, documentation of Individual Service Plans (ISPs), supervision of Support Coordinators, administrative tasks related to program management and supervision of staff, and the upkeep of office and center physical space.

At all times, the Program Director may be expected to assume the duties of an In-Home Support Coordinator (see Job Description, In-Home Support Coordinator). The Program Director will be responsible for hiring, training, supervising, and overseeing the duties of all Coordinators. The Program Director will maintain confidential employee files, supervision notes, and administrative actions related to those he/she employs.

ESSENTIAL PROGRAM DUTIES (other duties may be assigned):


Communicate effectively with DDS, individuals, family members, other employees and outside sources to be able to ensure comprehensive, well-rounded community services to those assigned to In-Home Supports.


Train new hires using the agency orientation checklist and document accordingly.


Maintain up-to-date knowledge of various social service entitlements (the RIDE, SSA/SSDI, SNAP, WIC, etc.) in order to assist individuals and other employees in accessing necessary supports.


Assist individuals with medication management utilizing the least restrictive measures and report any concerns to your immediate supervisor. Assure that direct reports are monitoring medications and upholding high levels of care regarding medication management.


Demonstrate effective use of Positive Behavioral Supports, Person Centered Planning, Trauma Informed Care and other Evidence Based practices as necessary.


Provide care coordination support and guidance to individuals in the following areas: educational, social skills, meal planning, clothing purchase and care, travel training, monitoring and assistance with personal funds, self-preservation, housing, community inclusion and religious practice if desired.


Transport individuals to medical appointments and coordinate follow-up medical instructions. Provide education to employees, individuals, family members, and providers about the benefits of self-determined and self-directed care to ensure the highest level of independence for the person possible.


Assist with healthy meal planning and purchasing; providing transportation to and assistance in the grocery store as needed to teach independent skills that allow for preference, cultural background, and dietary restrictions.


Inform individuals about social, leisure, educational, and developmental activities. Coordinate activities with individual input.


Maintain required documentation including, but not limited to, coordinator weekly notes, medical and historical records, ISP documentation, and program documentation. Ensure that documentation requirements relevant to the program are completed in a timely fashion or as assigned.


Position requires travel to and from various sites in personal vehicle. The Coordinator will provide safe transportation to individuals in personal vehicle as assigned.


Advocate for individuals human and legal rights. Assist individuals in learning about their individual rights and how to accurately and effectively advocate for their own wants and needs.


Provide assistance to individuals with everyday tasks as identified by the individual and the individuals ISP. Foster individual participation in these activities while maintaining a clean home environment. Assist with housekeeping tasks utilizing skill building tools. Utilize Person Centered Planning, Positive Behavioral Supports, and other Evidence Based Practices to encourage independence, increase skills, and build capacity.


Communicate and maintain good relationships with related service providers to exchange needed information to meet individuals needs.


Maintain open communication with family members, advocates, and guardians. Obtain and retain all necessary legal consents to be able to exchange information as needed, explaining the benefits of open communication to the individual and only relaying information to those from whom consent has been signed.


Attend in-services as assigned and trainings as required.


Act as Human Rights Officer at the request of the Program Director.


Maintain financial records of individuals funds as needed


Adjust schedule as needed/assigned to provide effective supports to the individual and the program


Communicate program concerns and issues to the Director of Family and In-Home Supports, in a timely manner.

Perform other duties as assigned.


Assist with creating, implementing, and maintaining an individual centered program based on the individuals preferences and availability


Maintain a cooperative team environment by working with all individuals within the program as assigned


Support the individual to maintain and increase independence in all aspects of their life utilizing the least restrictive measures whenever possible.


Provide Emergency coverage past the end of an anticipated shift due to emergency related to an individuals health, safety or well-being.

Planning/ Organization: Plans and manages work to accomplish tasks on time. Plans and implements appropriate use of resources. Adapts to changing conditions. Prioritizes work demands and manages multiple tasks simultaneously.


Responsibility: Adheres to company policies and procedures and follows company guidelines in conducting business relationships.


Attendance: Adheres to program schedule and routines, which includes arriving to assigned location promptly and conforms to the agencys attendance standards. Attend all required meetings and trainings promptly.


Initiative: Self-reliance in thinking, planning, and carrying out job responsibilities. Demonstrates problem solving skills.


Management: Supports and enforces agency policies and objectives; sets example through personal conduct and performance.


Data collection: Collects data, compiles information and prepares reports in a timely and accurate fashion.


QUALIFICATIONS:

  • Possess or be enrolled in a course of study culminating in an undergraduate or higher degree in human services; in certain instances, experience and other qualifications may be sufficient.
  • Successful completion of all state and agency mandated trainings.
    • First Aid/CPR Certification. (Within first month of employment)
    • Safety Care/Legal Certificate (Within first six months of employment)
    • DPPC/Fire Safety/Human Rights/Universal Precautions Certificate (Upon hire)
    • CORE Curriculum (Within first three months of employment)
  • Knowledge and use of principle of Normalization and Community Inclusion.
  • Knowledge and use of Positive Behavioral Supports preferred.
  • Must be willing to authorize a Criminal History Background Check (CORI) and release information concerning allegations from Disabled Persons Protection Commission (DPPC), fingerprinting, Department of Disability Services (DDS), and other agencies.
  • Must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable members with disabilities to perform these essential functions.
  • Drivers License that is valid in Massachusetts and reliable personal transportation.
  • Proficient computer skills in Microsoft applications.
  • MAP certification within first 6 months of employment

LANGUAGE SKILLS:

Must be fluent in English and have the ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write advanced reports and correspondence. Ability to communicate effectively with members served in their primary mode of communication, members under supervision, employees of the organization, families and members of the community. Ability to effectively present information individually and in small groups.

PHYSICAL DEMANDS:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing duties of the job, the employee is regularly required to sit, stand, walk, stoop, kneel or crouch. The employee is frequently required to use hands/fingers to handle objects, tools or controls and to reach with hands and arms. The employee must frequently be able to generate the force to maneuver up to 100 pounds. Assistance in lifting members will be provided whenever possible. Employee must be able to lift up to 30 pounds from floor to waist and 10-15 pounds from waist to overhead.
